Main duties of the job
You will be required to deliver Occupational Therapy assessments and interventions to young people who present with a range of developmental and complex mental health issues. These responsibilities will include:
Completing assessments, completing reports & OT assessments to assess the occupational performance needs of our client group
Use a person centered & recovery focused approach, you will support young people to achieve their goals, to enable them to live as independently as possible & to participate within socially valued roles
Identify outcomes agreed with YPs goals as part of MDT treatment plan, using specialist mental health & assessment tools with MHT
Develop and implement a therapeutic OT program
You will be expected to work very closely with staff teams to ensure incorporation and implementation of MDT treatment plan
Act as care coordinator, ensuring that effective care plans are created, communicated, actioned & reviewed on an ongoing basis, using person centered, evidence based practice
Promote and develop the provision of OT within Nestlings Care whilst considering statutory and non statutory services to provide meaningful and purposeful care for YPs & their family
To attend and participate, in MDT, CPA, Educational & Social Care meetings of cases on their load. Actively support and liaise with families, H & Sc professionals and agencies
To be an effective team member whilst working on own initiative whilst supporting and offering supervision to our OTAs.
About us
Nestlings Care Ltd is an independent organisation registered with CQC and Ofsted to provide specialist residential placements for young people and young adults with mental health difficulties. We offer intensive, Individualised community care with 24 hour mental health crisis management. We have identified that there are a number of young people and young adults with complex mental health difficulties who are either deemed unsuitable for inpatient hospital settings, have recurrent hospital admissions or are delayed discharges from hospital due to inadequate or delayed access to mental health services in the community. We offer specialist therapeutic placements with an in-house multi-disciplinary mental health team. We provide holistic care within the community in order to empower young people to be independent and enjoy a good quality of life.
We currently have 5 homes in Greater Manchester, 1 home in Pudsey and 1 in Chester.

To undertake new assessments, competing reports, OT assessments, under the direction, guidance and supervision of Lead Occupational Therapist, to assess the occupational performance needs of a defined client group.

To promote and develop the role and provision of OT within Nestlings Care Ltd whilst considering statutory and non statutory services in order to provide meaningful and purposeful holistic care for service users and their families.
To attend and participate, where required, in MDT, CPA, Educational and Social Care meetings of cases on their case load. To actively support and liaise with families, health and social care professionals and agencies.
To be an effective team member, whilst working on their own initiative.
