    Job summary

    Band 6 out of hours nursing sister required for the Overnight service covering the Stockport Community. You will be visiting patients in their own homes including residential homes at night providing unplanned palliative care, End of life support, catheter care, symptom control and any unscheduled nursing need required by the Stockport Community to help prevent a hospital admission.

    You will be a community nurse with a keen interest in Palliative care and working out of hours. You will be working within a very knowledgeable welcoming team of nurses and Health care assistants with a robust preceptorship period.

    You will be liaising with out of hours Medical team Master call as an when required acting as an advocate for patients and their families/Carers whilst also Liaising with the District nursing day teams.

    Main duties of the job

    We are looking for flexible, innovative practitioners' with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers.

    The post holder should have good organisational and assessment skills be flexible and wiling to cover the service between the hours of 22:30hrs , over a 7 day period. working with a well established team.

    This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable. The post holder will need to be a car driver and have a car available for work with a UK valid driving licence and able to upgrade care insurance to business use.

    Job responsibilities

    If successful this vacancy will require an enhanced DBS check at a cost of £ This cost will be deducted from your first Trust salary

    Essential

  • 1st level registered nurse
  • Desirable

  • Nurse Prescriber V100
  • Degree/Cert/Diploma Specialist Practitioner Nursing in the Home (District Nursing)
  • Knowledge

    Essential

  • Up to date knowledge of nursing issues
  • Demonstrate research based practice
  • Competencies within the scope of professional practice to meet service needs
  • Up to date knowledge of community nursing issues
  • Desirable

  • Ability to assess risk especially in relation to out of hours services relating to patients and staff
  • Experience

  • Responsibility for a defined area that includes assessment, planning and implementation of care
