    Description

    Salary: £33,277 p/a (NJC Scale Range Inc. LW + pension and other benefits

    Hours of work: 35 hours per week, excluding breaks. You will be expected to work a flexible shift system that will involve working between Monday to Sunday within the hours 8:00am to 10:30pm on a rota basis

    Annual leave: 34 days per year, including Bank/ Statutory Holidays (pro-rata for part-time)

    As Assessment and Resettlement Worker to hold a case load and deliver, in conjunction with other members of the team, the Passage House Assessment Centre service by carrying out comprehensive assessments; facilitating the rapid resettlement of all individuals who are referred to and accepted by the Passage House Assessment Centre service to available supported networks in their home area or where they have a proven link.

    • Conducting thorough, needs led assessments and co-produce informed, accurate and effective Routes off the Street.
    • Facilitate supported resettlement and advice through case management to the services and areas most appropriate for individuals to move sustainably off the streets from Passage House Assessment Centre. This includes accompanying clients to housing presentations and presenting their case to housing officers.
    • Provide appropriate advice and support to enable individuals to improve their health and wellbeing.
    • Maintain individual, up to date and accurate client data by inputting into the INFORM recording system.

    General responsibilities

    • In conjunction with the Assessment and Resettlement Coordinator to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
    • To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
    • To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
    • To ensure that all The Passage policies and procedures are being adhered to, particularly those relating to Code of Practice and Confidentiality.
    • To contribute to the effective implementation of The Passage's Diversity and Equality Policy as it affects both The Passage and its work with vulnerable adults.
    • To at all times undertake your role in a professional manner maintaining a high quality standard of work, and to always work in accordance with the aims, values and ethos of The Passage.

    Desired experience

    • Experience of undertaking comprehensive Assessments and Resettlement support to socially excluded clients.
    • Effective experience of working within a team and achieving strong and effective relationships with others in order to achieve organisational aims and objectives.
    • Ability of building and developing effective relationships with a vulnerable, disadvantaged or excluded client group.
    • Awareness of effective engagement and relationship building with clients and providing sensitive psychologically and trauma informed interventions to ensure individual's needs are identified and addressed.

    Desired knowledge

    • An understanding of relevant legislation, policy and practice, e.g. Housing Legislation, Rough Sleeping Strategies, Mental Health Act, Housing Act, Enforcement Orders, Central and Local Government Strategies.
    • To understand the issues involved in resettling street homeless individuals and the benefits and challenges of reconnection.
    • Effective knowledge of psychologically informed ways of working within a residential setting.
    • Knowledge and understanding of safeguarding procedures.
