    Job summary

    hours per week. The complex needs team are looking to recruit a senior mental health Practitioner to join their expanding team. This is an opportunity to become part of a developing service, working with people who experience co-existing mental health and substance use. The complex needs team is a multidisciplinary team, made up of Mental health nurse, social workers, doctors, support workers and senior drug workers, working across the Plymouth Alliance in a number of settings such as homeless hostels, VCSE partners and clients homes.

    The post holder will carry a small caseload of clients supporting to access services such as detox, rehabilitation, and working with clients to deliver harm reduction work, Motivational interviewing work and goal setting. The team has regular group supervision, adn CPD with regualr access to in house training.

    Main duties of the job

    The post holder will Carry a caseload of clients with co-existing mental health and substance misuse issues.

    Provision of case load supervision forBand 5 mental health practitioners and line management.

    To contribute to professionaldevelopment, appraisal, performance review and recruitment.

    The post holder will attend and lead community drop in clinics in the homeless provisions across the city.

    Be responsible for the management of caseloads.Carry out reviews of caseloads and support the process of recovery.

    The post-holder will take part in theclinical audit. This will include annual CPA and Health Records audits.

    The post holder will ensure that their clinicalpractice is based on evidence and good practice and in accordance with any NICEguidance

    Job description

    Job responsibilities

    To promote positive risk to allowclients independence and autonomy of choice.

    To liaise with other community resourcesand facilitate use of mainstream resources.

    To maintain a caseload of clients with asignificant degree of mental health complexity.

    To participate in the shift lead andprovide appropriate supervision.

    Practice within the legal and ethicalframework set out by mental health related legislation to ensure that serviceusers interests and well-being are at the fore.

    Liaise, advise, develop and maintaingood relationships with service users, carers, relatives and fellow health,social care professionals who are involved in the delivery of services.

    Engage in support and care plans forthose who have complex needs, high risk behaviors and risk assess and managethese effectively with the MDT.

    Work collaboratively with adult socialcare to identify and implement any adult social care needs.

    Participate in all activities aimed atkeeping professional skills updated and contribute to dissemination of thatknowledge and experience to all staff and students within your area ofresponsibility.

    Act upon referrals from secondary mentalhealth services, other primary care workers consultants and other statutorybodies by providing triage, assessment, implementation and development ofprograms of care to individuals, groups and families, who are difficult toengage in mental health services.

    Implement and participate in theformulation of all policy and procedures within the area of responsibility inconsultation with relevant others.

    Person Specification

    Qualifications

    Essential

  • Registration with NMC for RMN/ LD Nurse.
  • Occupational Therapy Degree HCPC Registration.
  • Social Work Degree HCPC Registration
  • Post Graduate Specialist Knowledge and/or experience in Community Mental Health setting
  • Desirable

  • Recovery

  • Management/coaching qualification.
  • Motivational Interviewing
  • On-going mandatory training
  • Supervisor and or assessor training in line with the University of Plymouth or willingness to complete this training.
  • Experience

  • 3-years post-registration mental health experience
  • Relevant experience of working with adults with a history of severe and enduring mental illness.
  • Minimum 12-months experience in community mental health setting or clear evidence of how candidate is able to fulfill the functions of a community mental health practitioner.

  • Experience at a senior level which includes managerial experience, staff supervision and operational issues
