    Job summary

    This post is open to nurses (all branches), midwives, and AHPs. If you would like to make a difference for adults, children, and young people with complex needs (learning disabilities, autism, dementia, delirium, and mental health) then this post is for you. This exciting opportunity to work with the complex needs team as a Complex Needs Liaison Professional, will give you the opportunity to support us to ensure patients with a complex need receive excellent patient experience each and every time.     Main duties of the job

    The role of complex needs liaison professional is an integral role in ensuring that patients with complex needs receive excellent patient experience. Duties include:

    Support the development of practice withservice leads and others as appropriate.

    Work closely with all members of the ComplexNeeds Team to ensure the teams objectives, that are aligned to the complexneeds forward plan and complex needs delivery group action plan, are met.

    Provide advice and support on the mentalhealth act to ward staff and support the receipt and scrutiny of Mental HealthAct Paperwork.

    Raise the profile of the health care needs ofpeople with a complex need across secondary care provision.

    Bridge the gap between community andsecondary services to enable better communication and access to healthcare forpeople with complex needs.

    Assess and advise accordingly on the issuessurrounding the needs of some people with complex needs, on their admission tohospital care settings while working alongside clinical staff.

    Facilitate the meeting of individualhealthcare needs for people with complex needs.

    Job responsibilities

    The postholder will work under the supervision of the Lead Professional for ComplexNeeds and co-ordinate their activities to ensure that an efficient service isprovided.

    Supportthe development of practice with service leads and others as appropriate.

    Act as aspecialist in practice development issues to improve care for patients with aComplex Need.

    Workclosely with all members of the Complex Needs Team to ensure the teamsobjectives, that are aligned to the complex needs forward plan and complexneeds delivery group action plan, are met.

    Provideadvice and support on the mental health act to ward staff and support the receiptand scrutiny of Mental Health Act Paperwork.

    Maintainexcellent communication networks and relationships within and externally to theTrust.

    Raise theprofile of the health care needs of people with a complex need across secondarycare provision.

    Bridgethe gap between community and secondary services to enable better communicationand access to healthcare for people with complex needs.

    Assessand advise accordingly on the issues surrounding the needs of some people with complexneeds, on their admission to hospital care settings.

    Facilitatethe meeting of individual healthcare needs for people with complex needs.

    Work inpartnership with people with complex needs, self-advocacy groups, and carersgroups and Wakefield learning disability partnership board in the development,implementation and maintenance of service provision and change.

    Person Specification

    Qualifications

    Essential

  • Registered Nurse, Midwife or Allied Health Professional.
  • Desirable

  • Recognised teaching qualification post-registration qualification or experience of teaching.
  • Independent Prescriber.
  • Experience

    Essential

  • Clinical experience caring for patients with one of the complex needs (Learning Disability, Autism, Dementia, Delirium or Mental Health).
  • Evidence of continued professional development.
  • Desirable

  • Experience of audit, research and evidence-based care.
  • Knowledge and skills

    Essential

  • knowledge into at least one of the Complex Needs.
  • Teaching / assessment skills Organisation and negotiation skills.
  • Excellent verbal, written, interpersonal and communication skills.
  • Evidence of audit and change management ability.
  • Computer literate.
  • Ability to motivate self and others.
  • Desirable
