Main duties of the job
The role of complex needs liaison professional is an integral role in ensuring that patients with complex needs receive excellent patient experience. Duties include:
Support the development of practice withservice leads and others as appropriate.
Work closely with all members of the ComplexNeeds Team to ensure the teams objectives, that are aligned to the complexneeds forward plan and complex needs delivery group action plan, are met.
Provide advice and support on the mentalhealth act to ward staff and support the receipt and scrutiny of Mental HealthAct Paperwork.
Raise the profile of the health care needs ofpeople with a complex need across secondary care provision.
Bridge the gap between community andsecondary services to enable better communication and access to healthcare forpeople with complex needs.
Assess and advise accordingly on the issuessurrounding the needs of some people with complex needs, on their admission tohospital care settings while working alongside clinical staff.
Facilitate the meeting of individualhealthcare needs for people with complex needs.

The postholder will work under the supervision of the Lead Professional for ComplexNeeds and co-ordinate their activities to ensure that an efficient service isprovided.
Supportthe development of practice with service leads and others as appropriate.
Act as aspecialist in practice development issues to improve care for patients with aComplex Need.
