Would you like to see a variety of patients each day so that no day is the same?
We are a busy happy team who welcome new staff members and encourage staff development and learning. We are seeking an experienced nurse to join us in providing care and treatment to adults living in the area who have long term or complex health needs. The scope of your role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients with long term or complex conditions. You will be working in a multi-disciplinary way alongside a range of primary care services including those from GP's, social and voluntary sector. You will be responsible for managing your allocated work on a day-to-day basis ensuring patients are prioritised according to clinical need and that assessments and care plans are recorded in line with the Trust's standards, essential for this role is experience with line managing staff, the co-ordination of a shift and supporting junior staff.
To support you within your role, Southern Health NHS foundation trust, have an excellent learning and development department, access to further develop your clinical skills and leadership development are available and encouraged.
Our service runs from , 7 days a week and therefore this position requires flexible working and all requests will be considered.
Access to a vehicle and possession of a full driving license is essential.
Main duties of the job
The Integrated Community Team model aspires to provide a responsive, safe effective caring service that is well led. The team is commissioned to reflect the needs of the local community. Service aims include providing a local point of access, proactive case management and adoption of a co-produced approach with patients and service users. The Integrated Community Team, whilst operating from a local hub, delivers a joined up approach across a specific locality, to deliver excellence in care and engages with all practices and wider stakeholders in the locality.

We are currently working closely with other NHS Trusts to integrate all our collective community, mental health, and learning disability services. Our new organisation, to be known as Hampshire and Isle of Wight NHS Foundation Trust, is set to launch in July 2024.
The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
