    Description

    Integrated care is all about providing better care and support to our patients, users and the people of Oldham by co-ordinating our services more effectively, with the focus on improving the health and wellbeing of people across the city.


    You will join our multidisciplinary Community Rehabilitation and Falls Team carrying out holistic assessments of patient needs and develop, implement and evaluate treatment programmes.

    There is a varied caseload to support adults and older people within their own homes, including care homes.
    Working with band 7 team lead you will support the delivery of an efficient and effective service.


    You will be an autonomous practitioner and work as a key member of the multidisciplinary team and supervising junior staff.


    You will have a broad range of experience managing medical, surgical, neurological and mental health conditions and ideally some community experience.


    The service is community-based and would require you to be able to travel to various locations across Oldham on a daily basis.

    BSc/MSc physiotherapy

    Knowledge, Training & Experience
    To demonstrate an awareness and understanding of supporting equality and valuing diversity within their role
