West Kent Primary Care would like to recruit a Cancer Care Coordinator on behalf of Tonbridge Primary care Network (PCN).
Tonbridge PCN serves a population of around 68,000people. It is made up of 5 GP Practicesand has a growing team of professionals with the aim of making healthcare moreaccessible in local settings. The teamis comprised of Pharmacists, First Contact Physiotherapists, PhysiciansAssociates, Cancer Care Coordinators, Paramedics and Social Prescribers whoprovide services across a range of practices.
This role will involve working on site from a GP practice in Tonbridge, with some opportunity for remote working.
Main duties of the job
This role will support the GP practice to achievie the NHS England Early Cancer Diagnosis Direct Enhanced Service by supporting multidisciplinary and practice teams in the coordination of pathways for patients with cancer. The Cancer Care Coordinator will work with patients ensuring they have the right support to understand their diagnosis and what the next steps are to help them during this distressing time. A major part of the role will be safety netting and checking that patients referred into the two week wait pathway have received an appointment with secondary care in an appropriate timeframe. The post holder will act as a point of contact for patients during their cancer journey signposting to services such as Macmillan Services.

Checking QoF for indicators for conversation and offer to support or information withinthree months of a patients receiving a cancer diagnosis.
Checking that the patient has a Cancer Care Review with GP within 12 months of a patientsdiagnosis.
At times supporting the practice administrative team with tasks and other responsibilitiesoutlined by the Practice Manager.
Support practices to improve local uptake of National Cancer Screening Programmes,understanding practice data for uptake and how screening is carried out, throughcommunity of practice which supports peer to peer learning events, and engagement withlocal providers.
Point of contact for patients during their cancer journey, or their relatives, to answerquestions and deal with problems that arise, linking in or signposting to services such as thehospital team, district nursing or Macmillan services, benefits agency as appropriate.
Create a system of checks to ensure the patients on the two week wait pathway are seen intime appropriate manner by secondary care.
