    Job Title: Primary Care Network- Enhanced Practice Nurse

    Location: Primary Care Network Practices

    Responsible to: NetworkManager

    Job description

    Job responsibilities

    Enhancedpractice nurses have the following key responsibilities in relation todelivering health services:

  • worksas part of a MDT within the PCN;
  • consultswith patients, their family, and the multi-professional team to undertakeassessments of patient need and devise and evaluate complex care plans;
  • evaluatesand analyses clinical problems using their clinical knowledge, seeking out andapplying relevant evidence, enhanced techniques, interventions, and equipmentto make clinical decisions;
  • deliversenhanced clinical care in the context of continual change, challengingenvironments, different models of care delivery, innovation and rapidlyevolving
  • technologiesusing analysis and their underpinning knowledge to manage complexinterventions;
  • teachesand advises patients and their families on how to manage their condition orsupport the multi-disciplinary team to do so;
  • participatesin clinical audits and research projects and implements changes as required,including the development, and updating of practice protocols / guidelines andprocedures locally;
  • workswithin national and local protocols where these exist;
  • recognisesboundaries of their practice and know when and to whom patients should bereferred;
  • usingthe principles of delegation, delegates work to other members of the MDT andtake responsibility for the delegated activity as appropriate;
  • demonstratesinitiative and are creative in finding solutions to problems;
  • holdsresponsibility for team performance and service delivery
  • providessupervision to trainee nurse associate, nurse associates and general practicenurses.
  • facilitateand support the development and delivery of innovative training and educationto staff, to improve competence and confidence, in line with best practice andguidance related to area of expertise.
  • participatein and/or lead formal training events promoting best practice in area ofexpertise.
  • provideprofessional and clinical leadership and mentorship, acting as a clinical rolemodel on the delivery of evidence-based practice.
  • adviseand support on the development and delivery of work streams relating to area ofexpertise as part of the PCNs strategy, where appropriate.
  • workproactively with key local and national stakeholders to develop more integratedcare pathways and holistic models of care, through dissemination of specialistskills and knowledge.
  • contributeto the delivery of local policy, procedures and protocols for the PCN toimprove identification, assessment, ongoing support for patients and theirfamily carers.

    Essential

  • Experience of nurse-led management of chronic conditions.
  • Experience of implementing protocols and clinical guidelines.
  • Experience of quality initiatives such as clinical benchmarking.
  • Specific General Practice skills / qualifications
  • Excellent communication and interpersonal skills
  • Ability to work as part of a multi-disciplinary team
  • Ability to work with independently
  • Ability to organise and prioritise workload effectively
  • Ability to exercise sound judgements when faced with conflicting pressures.
  • Ability to handle a busy and varied primary care workload;
  • Ability to offer support within a clinical team and share learning points in team meetings
  • Understanding of and commitment to upholding the Nolan Principles of Public Life

  • An awareness of current developments and initiatives in primary care.
  • Understanding of the pressures faced by practices and healthcare teams

  • Qualifications

    Essential

  • Registered with the Nursing & Midwifery Council
  • Evidence of other post registration at level 7 or above relevant to their area of enhanced practice.
  • Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
  • Experience of working within general practice and PCNs
  • Evidence of personal development
  • Enhanced DBS check (within 12 months)
  • UK work permit (if required)

  • Experience of working with other primary care providers; OOH/EA
