    Job summary

    CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting

    We are looking to recruit a Specialist Community Physiotherapist to join our Community Therapy team, part of the wider Community Adult Health Service. This is serving the Hertsmere locality in Hertfordshire.

    The role involves working with patients in their own home to provide rehabilitation services and promote independence.

    Successful applicants will be part of an enthusiastic, patient focused, holistic team treating patients in their own homes. Our Multi-disciplinary team comprises Occupational Therapists, Physiotherapists, Nurses, Healthcare and therapy assistants, as well as support from administrators.

    We work collaboratively with GPs, acute hospitals, social care, hospices and other third sector organisations. You will have the opportunity to work with patients with a wide range of clinical conditions.

    We have a well-established structure for clinical supervision and opportunities for practice development including in-service training. This will provide you with fantastic opportunities to enhance your existing knowledge and skills. You will also have opportunities to supervise junior therapy staff and students.

    Main duties of the job

    o Be an active member of the clinical team, providing a comprehensive therapy service to residents of West Hertfordshire referred into the Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional Build and maintain key relationships with referrers to the service, to facilitate appropriate referrals and an understanding of the role of the Physiotherapist and the Multi-professional teamo To deliver appropriate treatment in a variety of settings including the patient's own home, residential and nursing homes and day Be responsible for the supervision of junior staff, rehabilitation assistants and Physiotherapy studentso Actively participate in Continuing Professional Development to support your own development and the development of the service. Including participating in any audit or research being completed by the service

    Job responsibilities

    Complete comprehensive autonomous and multi- professional assessments. To develop and implement treatment plans for clients. These clients will present with an exacerbation of a frailty related condition and will likely have Multiple co-morbidities including neurological conditions, musculoskeletal conditions, osteoporosis, learning disability and/or mental health diagnosis using evidence based outcome measures Participate in the Assessment Process when appropriate, performing overview assessments and complex specialist assessments as appropriate; including the ordering of moving and handling equipment relevant to rehabilitation need. Perform overview assessments and complex specialist assessments as appropriate. To communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with trust guidelines for sharing of information and informed consent. Agencies include General Practitioners, Adult Care Services, Healthcare Professionals, voluntary agencies and residential homes. You will be responsible for gaining valid, informed consent for assessment, treatment and sharing information in compliance with legal and professional guidelines. To be able to interpret and follow the guidelines in circumstances where patients do not have capacity to provide informed consent.Please see attached Job Description and Person Specification for full roles and responsibilities.
