    Provider Support Assessor - Sheffield, United Kingdom - Sheffield City Council

    The Short-Term Intervention Team is the Councils in House Social Care Provider service which delivers support to adults enabling them to live in their own homes by promoting choice and independence in their daily routine. This is primarily following discharge from hospital or intermediate care settings but can also be to avoid hospital admissions.

    We are looking to recruit permanent Full time Provider Support Assessors.

    The role of the Assessor will work closely with other health professionals and colleagues, delivering individualised support, based on what each person needs, with continuous review of how needs are being met to maximise independence.

    In this role you will be expected to Assess for someone's care and support needs and write a robust care plan and Risk Assessment in the person's own home. You will need to understand reablement, goals, objectives and contribute to the reviews of support plans as required, ensuring the right level of support is delivered only for as long as is needed, advising the person and their carers of alternative options as appropriate.

    You may need to refer to other teams and assess for and order appropriate equipment.

    You need to be flexible and forward thinking as we are constantly improving and developing more effective ways of working with all our customers and stakeholders.

    We are looking for people who understand what good customer service is, can display sensitivity and tolerance, be able to listen actively and have good overall communication skills both written and verbal.

    The successful candidate will be able to demonstrate skills in applying a systematic approach to problem solving and co-ordination of tasks, a willingness to work as part of a team and with others and have knowledge of adult social care provision.

    Car driver preferred.
