- Supporting the surgery in coordinating all key activity, including access to services/care navigation, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
- Supporting digital initiatives.
- Ensuring patients are invited and have access to appointments for vaccinations including flu and covid boosters.
- Ensuring patients are educated around the different roles of health care professionals currently offering primary care services.

About the Role Bury St Edmunds Primary Care Network has an opportunity for a care co-ordinator to join our PCN, based at Guildhall & Barrow Surgery or Victoria Surgery, two of our 5 GP practices across the Bury St Edmunds area and be part of our senior clinical support team. This role is offered on a full-time basis, for hours per week, over 5 days, primarily Monday to Friday between the hours of 0830 and 1800hrs, with some weekend hours required subject to demand. In this pivotal role you will be working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service, working on requirements for the PCN Direct Enhanced Service Contract (DES). The main duties of the role include:If you have experience of care planning, with strong communication skills and excellent organisational experience, then we would love to hear from you.
