    Job summary

    Areyou a motivated, enthusiastic and experienced nurse with a passion fordelivering excellent care.

    Underthe leadership of our newly appointed Head of Community Services, as Wellbeing Practitioneryou will use you experience to support the needs of patients living with a lifelimiting illness, helping them to live as well as possible, for as long aspossible.

    Youwill report directly to the Wellbeing Team Leader and alongside a team ofnursing staff, healthcare assistants and volunteers to deliver a holisticservice.

    **Interviews will take place on Wednesday 22nd May 2024 **

    Main duties of the job

    The postholder will work within the Wellbeing team to support theneeds of patients living with a life limiting illness, helping them to live aswell as possible, for as long as possible. Responsible for monitoring acaseload of patients, 1-1 patient assessment and review, and facilitation ofgroups. The postholder will work alongside a team of nursing staff, healthcareassistants and volunteers to deliver a holistic service.

    Job responsibilities

    Main Responsibilities

  • Contribute to the planning, delivery and facilitation of 1-1s and groups within the Wellbeing at The Hive programme; a series of informative, therapeutic and supportive 1-1s and groups for patients living with a life limiting illness and those close to them.
  • Hold a clinical caseload of patients living with any life limiting illness, offering holistic assessment, review and identifying appropriate service for the individual.
  • Work alongside and as directed by Wellbeing Service Lead and the wider Wellbeing staff team, providing support to wellbeing assistants as a qualified member of staff.
  • Provide guidance and support to volunteers involved in the facilitation of Wellbeing led services, including coordination of volunteers across the service.
  • Take responsibility for one or more of the group programmes within the Wellbeing at the Hive programme of support.
  • Work alongside colleagues in the wider hospice community services and external group facilitators to ensure a consistent offer of support
  • Clinical Responsibilities

  • Complete triage, holistic assessment and review of patients on wellbeing caseload, ensuring service offer meets their current symptoms, needs and concerns.
  • Provide person-centred palliative care, allowing patients to be involved in the planning of their care and make choices about their end of life care.
  • Facilitate group and 1-1 support for management of common symptoms experience by those living with life limiting illness, such as fatigue, pain and breathlessness.
  • Observe clinical changes within patients, noting changes to stage of illness; liaising with other community professionals as and when required to meet need.
  • Initiate conversations about end of life care and decision, supporting patients in the development of their own advanced care plans.

    Empower patients to make choices about their care and future, enabling them to identify what is important to them.

  • Complete risk assessments and/or Personalised Emergency Evacuation Plans for patients, requiring assistance with mobility to access Wellbeing Services
  • Complete and contribute to risk assessment for group activities held within the Wellbeing at the Hive group programme.
  • Ensure nursing care delivered is in line with approved guidance and processes and that clinical competencies are completed when delivering care.
  • Ensure all care provided is culturally sensitive to meet the needs of patients across the locality.
  • Maintain accurate records, ensuring that they are clear, concise and legible, and are in accordance with the NMC Standards for Record and Record Keeping, using the EMIS database.
  • Represent the wellbeing team at clinical meetings such as handover, safety huddles, referrals meeting, clinical review meetings, and locality MDT as directed by the Wellbeing Service Lead and/or Head of Community Services.

  • Ensure a safe environment at all times by adhering to health and safety standards, contributing to risk assessments, and reporting all identified hazards, incidents, near misses and concerns using Vantage incident reporting system.
  • Follow safeguarding reporting and recording processes to ensure safety of patients, visitors, staff and volunteers.
  • To adhere to mandatory training as specified by role and work within Hospice guidance, policy and procedure
