    Team/Specialism:
    Senior Practitioner - Hospital Discharge
    84 per hour (assignment rate)


    Contract:
    Locum, 3-Months


    Hours:
    Full-time

    Enjoy the convenience of daily payroll and our reliable in-house payroll system. To supervise 3 Social workers covering UCLH hospital Discharge Service
    To supervise the out of borough Hospital Social Worker
    To act as Deputy for Team Manager when on annual leave etc
    To liaise with colleagues on discharge HUB and find solutions for residents of Islington being discharged in a timely fashion
    Advise on MCA and BI process where applicable
    To screen safeguarding cases and ensure that it is safe prior to residents being discharge from hospital
    Degree or equivalent in social work
    Current Social Work England registration
    Significant frontline experience in Adults Services within the UK
