    Job summary

    Anexciting opportunity has arisen within the Wakefield Community (MY Therapy)team to recruit an experienced and highly motivated Band 6 Occupational Therapistto join the neurology therapy team.

    The postis a fixed term contract for 12 months to provide backfill for internalprogression.

    MYTherapy is a large MDT (incorporating Occupational Therapists, Physiotherapistsand support staff) providing goal led rehabilitation to patients in their ownhomes as well as offering a variety of group settings and out-patient clinicappointments all within Wakefield District.

    Thesuccessful candidate will work within the community neurology team to providerehabilitation to individuals with a neurological disorder (including, Stroke,Motor Neurone Disease (MND) and other progressive neurological conditions)facilitating timely discharges from hospital, and support individuals to managetheir condition effectively to maximise function and prevent unnecessaryhospital admissions. This post will provide an opportunity to be involved inthe integration and delivery of health and social services to Wakefieldresidents in line with providing care closer to home.

    Main duties of the job

    The successful candidate will work together with colleagues, partners, patients and their families/carers in providing rehabilitation in the community thereby preventing unnecessary admissions to hospital / residential settings whilst also facilitating discharge and providing goal directed rehabilitation. We are an agile workforce and deliver care to patientsacrossthe Wakefield district.

    The successful candidate would be working predominantly within the patients own home or within care home settings, providing specialist occupational therapy intervention to treat a range of neurological conditions.

    Senior staff are expected to take an active part in the leadership of the team and offer support to junior and support staff and students. We encourage all staff to be instrumental in service development and alsooffer: -

    Key Responsibilities

    Clinical Duties/Patient Contact

  • To be professionally and legally accountable forall aspects of own practice, including the management of individuals in yourcare.
  • To effectively self manage a complex, multiplecaseload within the Neurology element of the Community Rehabilitation Service.
  • To know the professional and personal scope ofown practice and make referrals to other agencies.
  • To demonstrate specialist knowledge andunderstanding of the key concepts of biological, physical, social,psychological and clinical sciences in relation to Occupational Therapy inneurology.
  • Toprovide a rapid specialist Occupational Therapy assessment and comprehensiverisk assessment, using a high level of clinical reasoning skills to formulatean appropriate care pathway to prevent unnecessary hospital admission orfacilitate safe and timely discharges.
  • Tobe flexible to work in a variety of clinical settings Accident andEmergency, Fracture Clinic, Medical Assessment unit, Residential/Nursing Home,Resource Centres and in the community settings (patients home).
  • Tobe flexible to cover for the co-ordinator of Intermediate Care Service in timesof absence to act as a gate keeper to the services.
  • Toprovide a specialist Occupational Therapy opinion to the co-ordination team onthe suitability and appropriateness of potential patients/referrals to the IntermediateCare Service and particularly the neurology element of the service.
  • Followinga comprehensive specialist Occupational Therapy and risk assessment giveaccurate feedback and advice to relevant staff GP, medical staff, nurses,co-ordination, Physiotherapists, Social Workers, Health Care Assistants, ThirdSector colleagues and family/carers.
  • To be responsible for the assessment,prescription and evaluation of specialist equipment and adaptations forpatients with multiple and complex needs.
  • To demonstrate a detailed clinical knowledge ofa wide range of medical and neurological conditions.
  • To be able to use research, clinical reasoningand problem solving skills to identify the therapeutic benefit of client centredinterventions and make professional judgements with individuals with diverse orcomplex needs, or multiple problems according to outcome, changes in patientneeds and environmental influences.
  • To use specialist knowledge, training andexperience to plan and implement client centred individual and/or grouptreatment programmes for people with complex physical, cognitive, mental healthand social needs.
  • To be able to formulate specific and appropriatemanagement plans including the setting of time scales to accurately prioritiseneeds with individuals and the multi disciplinary team, in relation toavailable resources and professional demands.
  • To monitor, analyse, evaluate and modify OccupationalTherapy intervention and treatment in order to measure progress and ensureeffectiveness.
  • Using highly specialised knowledge, carry outand supervise a variety of physical interventions that require a high level ofphysical and manual dexterity.
  • To apply a high level of knowledge of the effectof disability and provide training and advice on lifestyle changes andadaptations to the individuals social and physical environment.
  • To demonstrate a detailed working knowledge of medicalconditions and current Occupational Therapy approaches to their treatment andapply a high level of knowledge of the effect of disability to provide trainingand advice on lifestyle changes and adaptations to the individuals social andphysical environment.
  • To work in a variety of environments, which mayinclude confined and unhygienic spaces that could present a risk to personalhealth and safety.
  • When necessary, to be responsible for theco-ordination of the individuals clinical care package.
