    Job summary

    An exciting opportunity hasarisen within the Breast Cancer Specialist Nursing Team.In the role as Clinical Nurse Specialist you willact as an advocate for patient care, providing seamless and timely managementof patients who may or may not have a diagnosis of Breast Cancer. This rolealso includes reconstruction clinics and breast screening assessment clinics.This fast paced and varied role requires anindividual who shows a methodical approach to patient care and able tocommunicate effectively with the team. Working alongside the Band 7 CNS and the widerteam you will help to support patients within a cancer pathway. You willprovide an individualised plan of care for patients by ensuring effectiveinformation gathering is performed, you will communicate effectively the planof care both within TRFT and also with other Cancer Hospitals involved in thepatients care. This is a permanent contract. The ideal candidatewill be able to work effectively with both patients and the wider MDT whilstworking cohesively with Breast Nursing Team in the discussion of patient care. This unique opportunity requires you to becommitted, organised and an excellent communicator with excellent verbal andwritten communication skills. You must be able to work as part of a team, helpto co-ordinate the caseload of patients using a patient centred approach whilstalso supporting each other in working within National Guidelines

    Main duties of the job

    The main purpose of the rolewill be to provide support for the breast screening assessment clinics, nurseled reconstruction clinics, in the ongoing development of the services whilstalso providing exemplary care to patients diagnosed with breast cancer on thecase load. The ideal candidate will be an experienced nurse working at Band 5/6with a keen interest in Cancer services. You must be able to show the ability to providespecialist holistic support and treatment whilst co-ordinating services andpersonalising the cancer pathway for the patients from their diagnosis, throughtreatment and also guide patients through the breast self-management programme. Be able to provide essential data collection tothe Band 7 CNS and be able to present this information as requested to others. Participate in audit, gather information and beable to input this on to various data collection forums. Act as an advocate for patient care, be able toliaise with multiple professionals across both TRFT and other cancer/generalHospitals, formulate treatment plans and communicate these effectively.
