Description
Job Summary
We are searching for a confident, highly motivated, and experienced Clinical Sister or Charge Nurse to join one of our integrated community nursing teams. You will collaborate with registered nurses and other health and social care professionals to provide assessment, care, and treatment to adults and older individuals living at home with complex and long-term physical health needs.
This role involves working independently within a dedicated nursing team, delivering evidence-based high-quality care to patients with life-limiting illnesses and offering support to their families.
You will need to showcase innovative clinical management and leadership skills, as well as mentor the continuous professional development of team members and assist in the growth of the Community Nursing Service under the supervision of the Locality Lead.
Main Duties of the Job
- Responsible for assessing, planning, implementing, and evaluating patient care in adherence to Walsall Healthcare NHS Trust policies and procedures to enhance the patient experience in the community - Provide high-quality palliative and end-of-life care and support to patients and their families - Offer professional support and mentoring to staff and teams, fostering continued professional development to enhance patient care - Maintain effective communication with the multi-professional team to ensure coordinated patient care - Support the implementation of the integrated care model and collaborate with key organizations to improve patient care

Walsall Healthcare NHS Trust is the sole provider of NHS acute care in Walsall, serving a population of 286,700. Our services range from inpatient and outpatient care at the Manor Hospital to various community services. The Manor Hospital hosts a wide range of district general hospital services, including a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit. Our ongoing development includes a new Urgent and Emergency Care Centre slated for completion this year.
