    About The Role

    The Dorset Council Adults and Housing Directorate delivers adult social care, housing, and a rangeof community services to people of all ages living in Dorset within the context of the Dorset CouncilPlan.

    The Council Plan is focused on making Dorset 'A great place to live, work and visit'. This will bedelivered through 5 priority outcome areas:Staying Safe and Well, Economic Growth, UniqueEnvironment, Suitable Housing, and Strong and Healthy Communities.The Directorate contributestothesecorporateoutcomes.

    The Adults and Housing Directorate has launched an ambitious transformation programme to ensureAdults, Housing and Community Services are strengths based, co-produced, can manage demand and are financiallysustainable.

    The Advanced Practitioner advisor role is responsible for the day to day leadership and management of the Council's CHC (Continuing Healthcare) Hub, ensuring the delivery of the responsibilities of the LocalAuthority within the Continuing Health Care National Framework.

    The post-holder will be supported by the Specialist Manager and will have specific responsibilities to support the professional development of the officers within the CHC hub which comprises qualified Social Workers and business support staff. The CHC hub has an important interface with the locality and specialist operational teams, supporting them with the CHC application and assessment process as well as with joint funding applications. The post holder will also ensure teams work collaboratively with housing, finance, performance, and commissioning colleagues as well as with Health partners within the Dorset Integrated Care Board. The post holder will need to take a lead in developing our collaborative and joint working practices with NHS Dorset in this area.

    The post-holder will support the designated staff with implementation of the Care Act, the NHS Continuing Healthcare National Framework and otherrelevantlegislation includingchangesassociatedwiththesocialcarereforms and CQC quality assurance framework. They will be leading developments in quality of practice, ensuring that practice is compliant with legislation and delivers the Local Authorities responsibilities. As part of this they will support the development of learningand development opportunities which should be aligned with professional developmentrequirements.

    The post holder must be prepared to represent Dorset Council at both operational and strategiclevels across NHS England and Integrated Care Boards in developing policy and improving collaborative working with partners across the local (and national) healthand social care system. The role will include responsibility for attendance at IndependentReview Panels.

    The post-holder will support the delivery of the practice and service changes required tomake Dorset Adult Social Care a modern and progressive service which meets theoutcomes of people in Dorset. They will role model through their leadership the behaviours' that are required for this transformation. The role will lead change to embed co production in practiceand valuing the diversity and experience of the adult and their family to improve theiroutcomes.

    It is expected that the post-holder will support the strategic development and implementationof specific workstreams within the transformation programme. They will contribute to the designand implementation of new operating models for adult social care.They will also contribute to the leadership of theDirectorate work, working closely with colleagues in other workstreams to ensure acollaborativeapproach.

    The post holder will be involved in audit work, quality assurance and budget meetings andwillprovide regularperformanceand qualityreports asrequired.

    The post-holder will be expected to participate in Directorate meetings and contribute towhole council working groups. They will be required to liaise effectively with local electedmembers and have an ability to work collaboratively with partner organisations within DorsetICBand awide rangeofstakeholders.

    Further Information

    This position has a significant travel requirement. This means that there is a requirement fora vehicle (or transport deemed to be suitable by the county council) to be available on mostworking days in order to carry out normal duties.
