Community Inclusion worker - Oakham, United Kingdom - Rutland County Council
1 week ago
Description
An exciting opportunity has arisen for a Community Inclusion worker to work with us in our Rise integrated neighbourhood team. The post holder will work closely with Primary Care, Rutland County Council, community health and Voluntary Sector colleagues. If you have experience of working in a health or social care environment, providing support to people within their own homes and community to enable them to live independently and reduce the need for health and social care interventions we want to hear from you.
About you...
About the role...
The community inclusion worker will undertake the following:
The community inclusion worker is based within the defined neighbourhood of Rutland, to improve joined up working practices leading to more efficient patient/service user care. As part of this role, the post holder will take on the administrative management of the regular multi-disciplinary team meetings and agreed actions from these meetings. The community inclusion worker will work with all health and social care professionals, as well as statutory/non statutory agencies, to provide a seamless, integrated service to our service users.
This role will also be considered as a secondment opportunity.
The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.
The community inclusion worker interviews will be held week commencing 27 May 2024. Please be advised this could be subject to change, in which case we will give appropriate notice.
Our offer to you:
Rutland Council is committed to Flexible Working and understands the benefits of supporting employees to manage their work and life balance, whilst meeting the needs of the service.
We are committed to supporting and empowering our staff to be the best workers that they can be and Rutland Council is the place you want to be. Our benefits go beyond the salary – being part of a great team, excellent Local Government Pension, generous annual leave, opportunities to learn and develop your career, and a demonstrable commitment to supporting the health and wellbeing of our staff.
Whilst the base for this post is Oakham, Rutland, the Local Authority offers a hybrid working practice. As well as working from home, we feel it is important that you have the opportunity to spend time in the workplace with your colleagues to help you learn, develop and engage.
