    Highly Specialist Pharmacist- Hepatology - London, United Kingdom - Guy's and St Thomas' NHS Foundation Trust

    Job Summary

    We are seeking an experienced senior clinical pharmacist to lead the pharmaceutical care for Hepatology patients at GSTT for a 12-month period starting in September 2024. The Hepatology service at GSTT focuses on the care of patients with liver, biliary tract, and pancreas disorders, aiming to provide exceptional care in an accessible and responsive manner.

    An exciting opportunity is available for a fixed-term (12 months) experienced pharmacist to lead the hepatology service for a specialist group of patients. You will collaborate closely with the consultant team to maintain and improve care pathways for patients with viral and autoimmune hepatitis.

    About Us

    The post is part of the Hepatology MDT, where you will serve as a clinician leading pharmacy services in this area. The MDT and Gastro Pharmacy Team are small and friendly groups that work together to solve patient and pathway challenges.

    Main Duties of the Job

    • Liaise with the South Thames ODN at King's College Hospital to treat patients with hepatitis C in line with UKHSA's goal to eliminate viral hepatitis by 2030
    • Collaborate with the GSTT Hepatology MDT to create tailored treatment plans for patients with Hepatitis, considering drug interactions
    • Monitor autoimmune/viral hepatitis patients on immunosuppressants and/or antiviral therapy through pharmacist-run prescribing clinics
    • Provide guidance to ward pharmacy and clinical teams on managing hepatology inpatients
    • Work with the Hepatology MDT to develop services, pathways, educational resources, and patient information to meet national targets and IQILs standards
    • Support the management of the Gastroenterology Pharmacy team, including IBD patients
    • Offer clinical support to both IBD and hepatitis patients through patient email helplines

    Job Responsibilities

    The ideal candidate will:

    • Have experience as a clinical practitioner managing patients with chronic diseases
    • Understand outpatient clinic pathways, preferably in hepatology or gastroenterology
    • Demonstrate a dedication to patient care
    • Act as a role model in providing pharmaceutical care to complex medical patients
    • Have the ability to influence at a consultant and directorate level
    • Show experience in service development in a multi-disciplinary environment
    • Possess strong leadership, facilitation, and communication skills
    • Have experience in training and supporting pharmacy and MDT staff
    • Hold a Postgraduate Diploma in Clinical Pharmacy Practice or equivalent
    • Possess or be working towards an Independent Prescribing qualification
