Main duties of the job
Thefollowing are the core responsibilities of the practice Develop,implement and embed health promotion and well-being programmes.Implementand evaluate individual treatment plans for chronic disease patients.Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health.Provideroutine nursing care to patients as required in accordance with clinical basedevidence, NICE and the NSF.Providewound care.Providetravel medicine services.Requestpathology services as necessary.Processpathology results as required.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary / specialist care as read codes are used effectively.Maintainchronic disease GPs with minor surgery when required. Supportthe team in dealing with clinical emergencies.Recognise,assess and refer patients presenting with mental health needs.Implementvaccination programmes for adults and children. Delegateclinical responsibilities appropriately (ensuring safe practice and the task iswithin the scope of practice of the individual). Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately.

Job descriptionJob responsibilities
Inaddition to the primary responsibilities, the practice nurse may be requestedto:
a. Actas the audit lead, effectively utilising the audit cycle
b. Monitorand ensure the safe storage, rotation and disposal of medicaments
c. Supportjunior members of the nursing team, providing guidance when necessary
d. Participatein local initiatives to enhance service delivery and patient care
e. Supportand participate in shared learning within the practice
f. Continually reviewclinical practices, responding to national policies and initiatives whereappropriate
Participate in the review of significant andnear-miss events applying a structured approach root cause analysis (RCA)
