Support practices with structured care around patients with long term conditionsFirst point of contact of care to establish a working diagnosis and management plan in partnership with the patient
Have direct and immediate access when needed to a GP and work under their supervision. Deliver integrated patient centred-care through appropriate working with the wider primary care MDT
Undertake consultations for emergency or routine problems and management of patients with long-term conditions
Undertake home visits, participate in duty rotas and possess a valid driving licence
Utilise clinical guidelines and partake in clinical audits, significant event reviews and other research and analysis tasks
A Post Graduate Degree in Physician Associate Studies from an accredited PA Programme
Experience of working in a medicine environment, including preventative medicine and chronic disease management
Flexible and adaptable approach to a varied and high-volume workload with the ability to multi-task and to work under pressure

Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited (DHPL), are developing new and exciting additional roles under the 2020 scheme, to support GPs and clinical teams at enhancing patient services in the local community.
The role will contribute to improving quality of patient care across the network of practices.We are delighted to announce that DHPL has been awarded the Herts Valleys Clinical Commissioning Group (HVCCG) Certificate of Achievement in recognition of exceptional contribution to the Primary Care workforce under the category of Supported Retention and Recruitment of Staff.
As such, you will be joining an enthusiastic team of clinicians and administrators and will be well supported with on-going professional development (training support, CPD and peer support).
You will maintain a key role in supporting delivery of network contracted services and to provide a holistic clinical service, with support from GPs as required.
Will have direct and immediate access when needed to a GP/Doctor and work under supervision of the GP with which the patient is registered.
Deliver integrated patient centred-care through appropriate working with the wider primary care multi-disciplinary team and social care networks.Undertake face-to-face, telephone and online consultations for emergency or routine problems, as determined by the PCN, including management of patients with long-term conditions.
Where required by the PCN, offer specialised clinics following appropriate training including (but not limited to) family planning, baby checks, COPD, asthma, diabetes and anticoagulation.
Undertake home visits and participate in duty rotas.Must possess a valid driving licence.
Provide health/disease promotion and prevention advice to patients.
Utilise clinical guidelines and promote evidence-based practice and partake in clinical audits, significant event reviews and other research and analysis tasks.
Through participating in continuing professional development opportunities, keep up to date with evidence-based knowledge and competence in all aspects of the role, meeting clinical governance guidelines for continuing professional development (CPD).
