    Job Title : Early Intervention Coordinator (12 month contract) *with possibility of an extension*

    Reports to: Team Leader

    Salary: Scale SO1, Points 29-31, £29,090 - £31,014 per annum, pro rata

    Hours of work: Full Time (37.5 hours per week)

    Key Responsibilities:

  • To assist young people to improve their life skills through designing and implementing life and social skills programmes in individual and group work settings.
  • To run a weekly drop-in center as part of our prevention strategy to offer early help and advice to young people and their families. To also sign post young people to relevant support agencies.
  • To set up and deliver outreach sessions working with a range of partner agencies and being the first point of contact for the customers that are referred to the service.
  • To be responsible for the development and implementation of enhanced partnership working across the public and the voluntary and community sectors to integrate the offer of housing advice, options and assistance and encourage joint work to provide the most relevant support to the customer across a range of support needs.
  • To monitor and review joint working arrangements and feedback on outcomes and performance to Youth Hub governance structures.
  • To provide advice and support to a range of stakeholders such as Service Heads, Social Workers, Youth Offending Service, drug and alcohol services, physical and mental health services, housing providers etc.
  • To develop and deliver training/ briefings to partnership agencies about the service and to be an ambassador for an integrated approach to working with young people to offer early help and their wider support needs at the earliest opportunity.
  • To work closely with other organisations and agencies to provide holistic multi- agency responses to housing and support needs.
  • To carry out homelessness prevention work, including liaising with landlords, families, and others, and by applying mediation and conciliation techniques wherever appropriate.
  • To manage and prioritise own caseload and evaluate each case, making decisions on appropriate options, meeting deadlines, and referring to other services and outside agencies as appropriate.
  • To safeguard young people and vulnerable adults at risk, through appropriate escalation and reporting to statutory services . Inter-agency referral and follow up work.
  • To ensure that the early help service is accessible to young people, that all relevant parties are informed of outcomes and that follow-up support is in place as and when required.
  • To attend panel meetings and review / case conference meetings regarding young people as and when required.
  • To keep up to date with legislative changes and other new developments relating to young people and early help resources etc.
  • Service monitoring-To submit monthly reports on drop-in service, number of sessions and activities undertaken.
  • To measure service impact through feedbacks and reporting on the grant outcomes.
  • To capture feedback from schools and other agencies.
  • To report on outcomes of support provision eg. Prevention of crisis, signposting, and referrals.
  • To support and encourage young people to undertake the St Basils life skills programme and to achieve the qualification.
  • To encourage and motivate young people to engage with and sustain education, training, and employment opportunities.
  • To support young peoples' progress, working innovatively and flexibly to assist with any problems which may arise.
  • To refer young people for opportunities in work experience and employment as appropriate.
  • Establish and develop links with training providers, employers, volunteering networks etc. in order to improve access to training opportunities for young people.
  • Identify and recommend additional resources and services appropriate to the client group.
  • Maintain accurate records of all young people action plans and progress.
  • To undertake holistic risk and needs assessments and to develop support and risk management plans.
  • To support and encourage young people to develop positive family and other support networks.
  • To prepare young people to take up and sustain settled accommodation and to continue to provide ongoing support to them.
  • To be available to young people for regular support sessions and to respond to crises.
  • To take responsibility for safeguarding the welfare of young people.
  • To promote and facilitate the participation of young people in their individual support plans and in the management of the service.
  • To facilitate young people's access to, and take up of, other support services.
  • To ensure benefit claims are made and maintained in order to maximise young people's income.
  • To organise and accompany young people to appointments as appropriate.
  • To maintain accurate records . support plans, day to day records.
  • To be flexible by being available out of hours on a rota basis to offer information, advice, and guidance to colleagues and in crisis situations, provide emergency cover within your own Project.
  • To work as part of a team, providing cover for colleagues as required by the line manager.
  • As part of a team to assist in the preparation of accommodation, which will involve some cleaning duties.
  • To assist in the induction of new staff, to supervise and support volunteers, trainees and students, as required by line manager.
  • QUALIFICATIONS

  • NVQ 4 or above in Advice & Guidance, Health and Social Care, Youth Work, or an equivalent relevant qualification.
  • NVQ Level 5, or working towards, in Advice & Guidance, Health and Social Care, Youth Work or an equivalent relevant qualification.
  • ROLE EXPERIENCE

  • Experience of working with young people in the following settings/types of work – Youth Work, Residential or Social Work, Probation, Community Work, Education and Training, Housing.
  • Experience of inter-agency working.
  • Experience of delivering training / briefings to those in authority and across multi-agency partnerships.
  • Minimum 2 years mediation experience working one to one with young people and families to prevent homelessness.
  • Working with Volunteers/Students.
  • Experience of designing and setting up referral pathways and co-ordinating multi-agency partnerships.
  • Experience of partnership development and sustainment.
