- Specialist assessment of individuals and their families
- Assessment and management of ongoing risks as identified during the assessment, ensuring that this is done in collaboration with service users and carers. Provide supervision of junior staff and trainees where appropriate.

- Deliver care under the Care Programme Approach and work in partnership with individual service users and their families and carers to ensure the delivery of the care plan.

- Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.

- Experience of partnership working within and across various statutory and non-statutory teams and agencies.

- Experience in working with people with dementia on the ward and in the community.
- A high level of working knowledge of cognitive impairment and dementia.

We are looking for dedicated, enthusiastic nurses who are highly motivated and caring professionals who can provide the best possible care with compassion to our patients.
You will have post registration experience, preferable, in the specified care group, and will be someone who can make a significant contribution in supporting the delivery of high quality assessment, investigation and treatment, supporting the team in the day-to-day running of the service whilst being expected to take a lead on a core aspect of service delivery.
The post holder will work with individuals who present with a range of memory related health needs, by providing assessment, information, treatment, review and support through individual and group work as part of a multi-disciplinary team.
You will work as a Care coordinator for a group of patients, and be responsible for liaising with other professionals and organisations to ensure that patients' physical, social and psychological needs are met in an integrated and holistic way.
Main duties of the job
The post holder will provide specialist assessments, reviews and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS). The post holder will ensure the successful implementation and roll out of care for dementia patients.
