    Job summary

    An exciting opportunity has arisen for an experienced Diabetes Specialist Nurse who is enthusiastic, passionate, highly motivated with a warm personality and great people skills.

    The post-holder will manage and facilitate a high quality, comprehensive service for all people with diabetes registered at the Practice, to ensure safe, effective delivery of high quality evidenced based diabetic care.

    You will need to be adaptable to change and be able to influence new ways of working, develop and implement strategies to improve outcomes and lead on diabetes training for all Practice clinical staff.

    You will be responsible for providing high quality evidence-based care for people with diabetes, support their families and carers, and have appropriate ongoing monitoring systems in place to ensure improvements are sustained, identified and actioned.

  • It is desirable that the successful candidate is working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
  • The post holder will be part of a team who provides high quality Diabetes nursing care.
  • Required to work flexibly to support the needs of the service.
  • We are looking for the successful candidate to work hours per week. We will offer a competitive salary dependent on experience and qualifications and 5 weeks holiday plus Bank holidays. We provide ongoing supervision, training and support from a welcoming team.
  • Main duties of the job

    The core responsibilities of the role are covered in full within the job and person specification.

  • Support patients with clear patient-centered individualised care plans agreed and tailored to each person with diabetes, by working closely with the Lead Diabetic GP, colleagues and members of the MDT.
  • Direct patient contact, giving highly specialised advice to help manage their condition. Triage referrals, undertake skilled assessments, manage own caseload, refer and discharge patients taking responsibility for the delegated activity. To see patients as required within the GP surgery, on home visits, care homes or advise and triage where appropriate on the telephone.
  • Deliver enhanced clinical care, challenging environments, managing ongoing change, different models of care and evolving technologies.
  • Perform invasive procedures relevant to the role and within competency. Process and act upon clinical results and tests.
  • Advise, support and provide training to newly diagnosed or long-standing diabetes patients and their carers. Also proactively identify, diagnose, and manage treatment plans for patients at risk of developing diabetes.
  • Diagnose, prescribe and manage medicines, providing information and advice on prescribed and over-the-counter medication, including side effects and interactions. Review medication for therapeutic effectiveness in line with evidence-based practice and national and Practice protocols.
  • Provide audits, QoF data capture, attend MDTs, refer patients.

    Newgate medical Group is a welcoming and busy GP Practice located in Worksop, Notts, providing regulated NHS services to our patients.

    We are a single site PCN with 30,000 registered patients. We have an established team of 9 GP Partners and a host of varied health professionals, employing over 80 staff, including pharmacists and 2 pharmacy technicians, assisting with medication reviews. Experienced and well developed nurse, HCA, management and admin teams, 3 trainee ACPs, PA, ANP & ACP undertaking home visits and care home reviews. Also social prescriber link workers, Care Co-ordinators and excellent integration with community nurses and the voluntary sector.

    Job description

    Job responsibilities

    The following are the core responsibilities of the Diabetes Specialist Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

    Promote clear patient-centred, individualised care plans that are agreed and tailored to each person with diabetes.

    Consult with patients, their family, and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans.

    Be involved in direct patient contact, giving highly specialised advice, support, and counselling to those newly diagnosed or with long-standing diabetes. Teaching and advising patients and their families on how to manage their condition or support the multi-disciplinary team to do so;

    Deliver enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using analysis and your underpinning knowledge to manage complex interventions.

    Interpret results (biomedical, blood glucose monitoring and continuous blood glucose monitoring) analyse and initiate appropriate action.

    Triage referrals, undertake skilled assessment, manage own caseload, refer, and discharge patients to other team members and specialist departments. Takes responsibility for the delegated activity as appropriate.

    Perform invasive procedures relevant to the role and within competency.

    Act as the patients advocate.

    Diagnose, prescribe, and manage medicines according to own competency. Provide information and advice on prescribed and over-the-counter medication, including side effects and interactions.

    Assess, diagnose, plan, implement and evaluate interventions/treatments for diabetic patients with complex needs, working independently but seeking advice where appropriate.

    Proactivelyidentify, diagnose, and manage treatment plans for patients at risk ofdeveloping diabetes.

    Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and Practice protocols and within the scope of practice.

    Work with patients to support compliance with and adherence to prescribed treatments.

    To see patients as required within the setting of the GP surgery and on home visits or advise and triage where appropriate on the telephone.

    Work with a high degree of autonomy (whilst working within your level of competency), responsibilities to include clinical care and patient education. Recognises boundaries of their practice and know when and to whom Patients should be referred.

    Be responsible for the clinical initiation and monitoring of insulin regimes or other injectable therapies, administering where necessary as a part of teaching, and encouraging self-management.

    To treat and refer appropriately, order diagnostic investigations, prescribe medication when necessary to create individualised patient care plans.

    Process and act upon blood test results, including HbA1c, cholesterol and C&E.

    Process and manage the blood pressure data.

    Review patient care and medication when asked by GPs

    Clinically assess, manage, and refer to specialist weight management programmes where necessary.

    Assess, treat, and implement treatment programmes for the management of wounds, taking swabs where necessary.

    Responsibility to work in close liaison with specialist Tissue Viability and podiatry services where necessary.

    Proactively identify and manage treatment plans for patients at risk of developing complications associated with diabetes and manage these patients.

    Keep abreast of the diabetic QoF domain as it changes and offer interventions to improve the clinical outcomes.

    Actively implement data capture for the QoF as indicated by the SystemOne alert.

    To take on a lead role within the QoF clinical domains of diabetes ensuring the recall and review of patients as appropriate.

    Ensure follow up, actively participate, and lead in the recall system for patients with diabetes.

    Maintain continuity of care with your patients and undertake proactive case management, taking ownership as diabetic clinical care lead.

    Attend MDT meeting and in house diabetic meetings to review the diabetic dashboard.

    Implement changes made when patients attend Secondary Care.

    Refer to Secondary Care and/or other services as required.

    Covering for holiday, training, and sickness absence for Nurse colleagues.

    Participates in clinical audits and research projects and implements changes as required , including the development, and updating of practice protocols/ guidelines and procedures locally.

    Works within national protocols where these exist.

    Demonstrate initiative and creativity in finding solutions to problems.

    Secondary responsibilities

    In addition to the primary responsibilities, the Diabetes Specialist Nurse may be requested to:

    To maintain effective liaison with the Practice Manager on all areas of responsibility and to immediately report any difficulties or shortcomings in agreed standards and criteria.

    Undertake training where relevant to the tasks and responsibilities associated with the job and under the direction of the Practice Manager.

    Perform any duty specifically delegated by the Practice Manager as being properly the responsibility of this post.

    To conduct oneself in a professional manner maintaining, courtesy, discretion, and patience always.

    Hold responsibility for team performance and service delivery; and provide supervision to trainee nurse associate, nurse associates and general practice nurses.

    Facilitate and support the development and delivery of innovative training and education to staff, to improve competence and confidence, in line with the best practice and guidance related to area of expertise.

    Participate in and/or lead formal training events promoting best practice in area of expertise.

    Provide professional and clinical leadership and mentorship, acting as a clinical role model on the delivery of evidence based practice.

    Advise and support on the development and delivery of work streams relating to area of expertise as part of the PCNs strategy, where appropriate.

    Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models or care, through dissemination of specialist skills and knowledge.

    Contribute to the delivery of local policy, procedures, and protocols for the PCN to improve identification, assessment, ongoing support for the patients and their family carers.

    Qualifications

    Essential

  • Qualified Nurse.
  • Registered Nurse (Nursing and Midwifery Council.
  • Additional training in diabetes.
  • Independent prescriber.
  • Relevant nursing/health degree/RNDA.
  • Desirable

  • Teaching/mentoring qualification.
  • MSC or equivalent - post graduate Level 7 in diabetes.
  • Experience

    Essential

  • A minimum of 3 years' experience as a qualified nurse.
  • Evidence of appropriate continuing professional development activity.
  • Proven ability to evaluate the safety and effectiveness of their own practice.
  • At least 2 years' experience in diabetes management.
  • Experience of prescribing and undertaking medication reviews.
  • Audit and MDT experience.
