    HFEHMind is committed to working in partnership with the NHS & other MentalHealth providers to ensure more Children & Young People can access MHtreatment & specialist support.

    Wetailor our response with appropriate services like talking therapies, peersupport, advocacy, crisis care, employment & housing support. In responseto government plans for expansion & provision an exciting Crisis Caféreferred to as Circle was developed by HFEH Mind, part of the new Complex NeedsHub.

    Thepurpose of Circle is to help alleviate mental health & wellbeing duringheightened arousal or crisis with supportive therapeutic provision in the localcommunity facilitated by the Circle team.

    The post holder will be a link between the Youth Services dayservices & Crisis Prevention services working with CYPs aged 518 who present or are referred to this crisis service.

    The role would suit a Health Care Professional who can be aqualified Nurse/School Nurse, Social Worker, Occupational Therapist, CYPWellbeing Practitioner, Educational Mental Health Practitioner with a min 2years post qualification & some training in CBT interventions,experience of working within a multi-disciplinary CYP MH team with experiencein managing CYPs presenting in a MH crisis/acute MH presentations with aninterest in developing their crisis response therapeutic practice & workingcreatively to provide effective evidence based de-escalation in non-clinicalsettings.

    Main duties of the job

    The post holder will mainly deliver short-term support for CYPs in crisis & will carry out screenings, risk assessments, safety plans & interventions using evidence based therapeutic treatment modalities on- site. You will also work to ensure a seamless onward transition to self-management, relevant specialist Community Mental Health services, Social Care or other relevant partner agencies where required.

    Job responsibilities

    Key Responsibilities:

  • Be involved in the development of an outcomes framework, evaluation, and practice in relation to service users mental health to include assessing risk and protective factors using individual, family, learning environment, and community domains to ensure we offer a highly responsive and adaptable service.
  • Support CYPs experiencing some form of Mental Health crisis that may find a less stimulating and highly supportive environment quite beneficial. You will assist in the delivery of evidence-based 1:1 and group interventions informed by a variety of treatment modalities like CBT, DBT and Systemic practice in the HUB.
  • Work with people from different cultural backgrounds using interpreters when necessary and as such should be committed to inclusivity and managing diversity as a typical part of your work ethic. You will also work closely with colleagues and partners in the Local Authority, CAMHS, education settings and other relevant mental health providers and should be committed to developing and fostering professional relationships.
  • Coordinate an active allocation of children and adolescents presenting in crisis with mild moderate emotional and mental health difficulties and will deliver evidence-based individual and group interventions in the Circle. This will include the delivery and coordination of interventions directed at emotional regulation for CYPs in acute crisis; co-produce individualised crisis plans and facilitate relevant psychoeducation with CYPs and parents/carers. These therapeutic interventions will have a psychosocial scope, be aimed at stress reduction, have a low stimulus therapeutic approach to mental health and be typically facilitated in the Circle.

  • Other professionals working with children and young people in local CAMHS, social services, A&E, education, youth offending and the voluntary sector.
  • Other HFEH Mind departments or services to include Human resources, Safeguarding and Adult services.
    • Service Delivery
  • Work as part of a multi-disciplinary team by providing containment and care to service users attending the Circle under the direct or indirect supervision as required.
  • Facilitate the support of children and young people, their parents/carers, families referred or presenting to the Circle in the self-management of presenting difficulties.
  • Provide practical 1:1 support through onboarding and telephone engagement as required.
  • Assist in implementing the delivery of programmes of care by working in partnership with children, young people, their families and other agencies in the development of intervention plans and monitor outcomes.
  • Design training materials and co-facilitate group or workshop delivery to promote emotional regulation, crisis identification and management, mental health awareness and self-care.
  • Awareness of community resources and facilitate the appropriate signposting or referrals for service users where the need for additional support or a different response is identified and agreed with CYP, CYPs parent or carer.
  • Facilitate the delivery of workshops and peer discussion groups as required.
  • Recognise safeguarding situations; be aware of and adhere to safeguarding guidelines; know when to inform and share these concerns with the Circle team and raise safeguarding alerts.
  • Observe, report and appropriately respond to any changes in the service users physical, mental health and behaviour and discuss them with the Circle team in consultation and as part of handover huddles.
  • Support other HFEH Mind Youth Service delivery when required and agreed by Team Manager.
  • Facilitate the development of Circle service tools, materials and services including carrying out audits and preparing service data for stakeholder presentation, training, and wider service presentations.
  • Facilitate the assessment, planning, implementation, and evaluation of crisis interventions under direct supervision of the Team Manager.
  • Participate in the regular case and service reviews or discussions and have key worker responsibilities for cases.
  • Initiate, organise, and evaluate practical support with therapeutic/clinical advice under the guidance of the Team Manager.
  • Utilise skills flexibly and effectively so that interventions and practical support can be creatively thought about and facilitated in the Circle or other appropriate settings.
    • Service Accountability:
  • Monitor and evaluate the effectiveness of the services delivered under the guidance of supervisor and the Team Manager.
  • Contribute to gathering and presenting information for quarterly monitoring reports.
  • Maintain records of clinical activity in accordance with wider and local service policies and procedures by keeping administrative processes, databases, and client records up to date.
  • Maintain accurate and timely information, documentation, and reporting systems in accordance with agreed standards, HFEH Policies, Circle Procedures and the Data Protection Act.
  • Awareness of the HFEH Mind Complaint Policy in order to promote excellence of service and consumer satisfaction.
  • Work within the constructs of and adhere to HFEH Mind and Circle agreed Covid-19 protocols/requirements.
  • Adhere to established policies, procedures, protocols within the service and those specifically relating to HFEH Mind and the Circle Team.
  • Appropriately and without undue delay seek support and consultation regarding complex Circle cases or new referrals.
  • Assess risk presentations, plan for and manage potential risk, respond to risk needs, document risk, case consult and seek guidance on the management of risk presentations and escalate risk to ensure needs are best met.
  • Carry out mental health & risk assessments on children & young people in your care routinely and work to manage these situations effectively under supervision.
  • Discuss new assessment in the MDT team meeting and collaboratively decide on an appropriate treatment pathway.
  • Maintain an awareness of current developments in the field of CYP mental health and managing Crisis by attending training courses and workshops and accessing up to date literature.
  • Participate and facilitate regular shared learning activities and developmental experiences across CAMHS.
  • Participate and facilitate service initiatives designed to improve efficiency and outcomes for children, young people and their families.
  • Ensure that all assessments and interventions are provided from an inclusive values base which recognises and respects difference.
  • Maintain comprehensive records of all training and intervention activity
  • Complete all requirements relating to data collection and report clinical outcomes and service access data into the digital patient record system, CYP IAPTUS.
  • Monitor and track progress against access targets for your caseload and develop plans with your supervisor to reach access targets
  • Develop a local knowledge of the locality and its services and facilities, particularly in relation to mental health issues.
  • Attend relevant educational opportunities in line with identified professional objectives
  • Carry out additional tasks as required by the Circle Team and core HFEH Mind service.
    • Partnership Working:
  • Liaise with and develop positive partnerships and relationships with other services.
  • Escalate/signpost referrals for children and young people with more complex needs to relevant services.
  • Maintain positive working relationship with partner agencies providing specialist advice and support where appropriate.
  • Communicate complex and sensitive information effectively with patients, relatives, and carers, where there may be barriers to understanding. Ability to deal effectively with patient or carer concerns, complaints, and anxieties as they arise.
  • Work autonomously and as part of a team to deliver and develop patient centered, evidence-based care as appropriate, using a range of advanced skills and knowledge for patients with both diagnosed and undiagnosed conditions.

  • Engage in regular group and 1:1 supervision clinical and management supervision and participate in appraisal processes as required.
  • Attend handover meetings; case review meetings and/consultation; regular team and handover meetings as required.
  • Travel across boroughs within London (predominantly Hammersmith & Fulham, Ealing and Hounslow) where required and agreed with Team manager for the purpose of Team representation, redeployment or specific service user needs.
  • Attend mandatory in-house and relevant external training opportunities, to support own ongoing development.
  • Facilitate Band 4 and volunteer staff supervision, appraisals and case consultation as required by the service.

    Experience

    Essential

  • Minimum 18 months working children and/young people in a mental health service as an HCP with relevant professional registration in one of the following:
  • Registered General Nurse (RGN) -School Nurse
  • Registered Mental Health Nurse (RMN)
  • Registered Learning Disabilities Nurse (RLDN)
  • Occupational Therapist (OT) -MH work experience.
  • Social Worker (SW) -MH work experience.
  • Experience in delivering programmes and workshops to groups
  • Experience in delivering 1:1 interventions/practical support to CYPs.
  • Working knowledge of child safeguarding
  • Experience of working with parents and carers.
  • Organisational skills and excellent time management.
  • Experience of using Microsoft Office package
  • Working understanding of General Data Protection Regulations (GDPR)
  • Positive attitude towards working with people with mental health problems.
  • Demonstrable knowledge and understanding of equality and diversity issues and the ability to integrate into employment policies and practices
  • Ability to communicate clearly both verbally and in writing.
  • Ability to organise and prioritise workload.
  • Ability to work flexibly according to the needs of the service and to support work/life balance and improving working lives.
