    Job summary

We aim todeliver an effective, high-quality service that puts patients at the centre oftheir own care. The team ethos is to enable and support peopleto remain in their own home as the first alternative to inpatient admission.

    The Neighbourhood Team consists ofPhysiotherapists, Occupational Therapists, Community Nurses, Senior Nurse Practitioner, HealthcareAssistants and administrators. GP and social care colleagues work closelyalongside.

    TheNeighbourhood Team operates from 7:30am to 18:30pm on a seven-day rotaincluding weekends and Bank Holidays. You will be supported to work flexiblywithin these hours.

    Main duties of the job

    Yourtypical working day will include triage and allocation of planned and unplannedvisit requests for physiotherapy interventions. This may involve urgentresponse assessment as part of the teams admission avoidance remit, supportingcomplex hospital discharges and planning domiciliary interventions to facilitateindependent living. You will work alongside physiotherapy, nursing and OTcolleagues in the same team. We have close links with Worcester and Birminghamuniversities in support of student placements.

    Job description

    Job responsibilities

    Working as an autonomous practitioner withinan integrated team you will provide timely and expert professional assessmentand treatment interventions to patients either in their own homes or othercommunity settings. This will be achieved through a combination of hands onexpert clinical skills and knowledge as a Physiotherapist to treat, educate andsupport patients and by working with patients and their families to assess, planand implement appropriate care plans and treatment.

    Providecase management for a designated caseload of patients who have immediate and /or complex care needs.

    Triage patients to ensure prioritisation ofclinical need.

    Develop and co-ordinate individualisedrehabilitation plans in close liaison with the team, the patient and theircarers as well as all other services.

    Work holistically with all team members,carrying out joint visits and actively participating in the integration of services - increasing the knowledge of all team members through case discussion and joint working.
