    Job summary

    The role of the Enhanced support nurse is to work as part of an MDT to provide intensive support to those people with learning disability who are experiencing mental health or behaviour crisis. The team provides a 7 day a week service to support people with learning disability to remain safe in the community and avert the need for admission to hospital. Where hospital admission is required you will support the inpatient team to plan for a timely and safe discharge back to the community and provide short term follow up care.

    Main duties of the job

    Liaising with mainstream mental health services; other stakeholders including health and justice teams and police to support the service user to achieve their optimum level of independence.

    To provide education and training to other providers in the management of mental health and behaviours that challenge. Supporting them to provide holistic healthcare that incorporates evidence based therapeutic interventions.

    To support the STOMP clinic and promote the clinical care pathways implemented by Essex Learning Disability Partnership.

    The successful candidate will need to be able to network and develop close working relationships with commissioners and other providers to ensure that our service users are given the most appropriate support and are given equal access to the services they require.

    About us

    Essex Learning Disability Partnership provides specialist learning disability services across all Essex. The partnership has been formed by Hertfordshire Partnership NHS Foundation Trust providing Learning Disability Services in Mid, North East and West Essex and; Essex Partnership University NHS Foundation Trust providing Learning Disability Services in South Essex.

    The Partnership was formed in 2018 and has been developing new pathways of working that seek to improve health outcomes for people with LD making sure that they are able to access the right care in the right place at the right time, in accordance with Building the Right Support. The successful candidate will have the opportunity to be part of this continuing journey and influence how we work to improve lives.

    The learning disability service was awarded outstanding for caring in our last CQC inspection and good in all other areas. Team members have won awards for innovations in practice and mentoring students. The Clinical Lead Nurse has been awarded the title of Queen's Nurse and the STOMP clinic set up by two non-medical prescribers has been recognised by NHSE as an example of best practice.

    The successful candidate will also have the opportunity for continued personal and professional development including, but not limited to Non-Medical Prescribing.

    Job responsibilities

    The post holder will:

  • Work as a practitioner with interest in challenging behaviour and mental health wellbeing for people with learning disabilities.
  • Support the Advanced Clinician in the development of the EST service
  • Support the smooth running and coordination of the service and monitoring service activity.
  • Be responsible for maintaining clinical standards under the direction of the Advanced Clinician.
  • Work in liaison with services users, carers, other professionals ensuring the delivery of a needs led service
  • The post holder will have the clinical skills relevant to the delivery of care in the service area or demonstrate an aptitude to develop these skills.
  • The post holder will take responsibility for formulation of appropriate care plans for those service users presenting with challenging behaviour or mental health crisis.
  • The post holder will be able to assess service users thoroughly, plan care rigorously, audit, and evaluate interventions based on evidence and locally agreed protocols and policies.
  • The post holder will act in a consultative role to external agencies in relation to clinical interventions, risk assessments and management plans.
  • The post holder will ensure they are fully conversant with all aspects regarding the service user care and treatment.
  • The post holder will offer professional support to junior members of the Enhanced Support Team in accordance with professional responsibility.
  • The post holder will provide interventions in a person centred way that promotes and facilitates the positive health and wellbeing of people with learning disabilities.
  • The post holder will maintain close liaison with all departments involved in a service users care and report on progress and effectiveness to the MDT.
  • The post holder will participate in reviews within the community and inpatient settings and will prepare reports on service users as required.
  • The post holder will be aware of all policy documents and legal requirements pertinent to the post and ensure that these are followed.
  • The post holder will lead specific groups and sessions with clear objectives.
  • The post holder will ensure that the statutory requirements of the Mental Capacity Act and Mental Health Act are observed and applied.
  • The post holder will provide specialist education and training regarding learning disability for those people working within local health care/social care/residential care/third sector settings.
  • The post holder will carry out clinical procedures when required in accordance with trust policy and their own competency.
  • The post holder will act as a practice assessor and practice supervisor to students.
  • The post holder will undertake clinical risk assessment and develop with these with comprehensive multi-agency contingency planning.
  • The post holder will support in the development of evidence based quality practice.
  • The post holder will provide training in response to specific service user need.
  • The post holder will communicate clinical formulations, hypotheses, current problems, contributing factors to service users, families, advocates, care providers and other stakeholders.
  • The post holder will provide clinical supervision for junior staff.
  • The post holder will monitor delegated tasks to unqualified staff as part of direct and indirect supervision.
  • The post holder will promote the inclusion of people with learning disabilities into mainstream services ensuring that they have access to and are included in service development and improvement planning.
