Description
The post holder will be an autonomous practitioner managing a caseload of patients presenting through the Lower Suspected Cancer referral service.
The CNS will be expected to deliver nurse-led clinics (face to face and telephone) and supporting patients on the 62 day pathway.
The post holder will work alongside the Senior Colorectal Nurse Practitioner, Lead Colorectal Cancer Lead, Cancer Management Team and Colorectal Consultants providing clinical excellence for all patients with possible cancer.
To work according to the NMC Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker (with facilitation) for a defined group of patients and take responsibility for:
The assessment of care and health education needs
~ The development, implementation and evaluation of programmes of care including discharge planning for each patient.
~ To work in accordance with the RUH Nursing Strategy and contribute towards achieving its objectives
~ To work closely with the surgical/ gastroenterology team to ensure that the resources available are used effectively and to maximum capacity.
~ The post holder will be expected to work in a flexible manner for which may include working evenings and at weekends.
~ To develop an educational and counselling role as part of overall patient care.
~ The post holder will be expected to work a proportion of their time within the outpatient department leading on nurse led clinics as well as telephone nurse led clinic
~ To assist in establishing and monitoring protocols/care pathways and working with the Senior Nurse Practitioner to develop and review the 2WW Service.
