    Detailed job description and main responsibilities

    1. To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, BAOT, HCPC and the RCOT Code of Ethics and Professional Conduct.
    2. To ensure that the client and or their carer has an understanding of treatment protocols and gives informed consent to assessment and treatment.
    3. To set and maintain a high standard of clinical practice which is evidence based where possible, using standardised assessment and outcome measures, where appropriate
    4. To undertake specialist occupational therapy assessment of clients with diverse needs through subjective questioning, objective assessment and other written material available.
    5. To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social emotional and mental health issues.
    6. To set goals for treatment with clients and/or their carer and ensure that care is client centred and agreed by the client and/or their carer.
    7. To set and maintain a high standard of clinical practice which is evidence based where possible, using standardised assessment and outcome measures, where appropriate.
    8. To independently screen and assess complex multi-disciplinary referrals/care plans from a variety of sources. This screening requires individual responsible action when at times no other professional is currently involved. This screening will establish priority, determine needs and initiate a plan of intervention.
    9. To manage a caseload of clients with simple to complex needs with supervision and guidance as necessary from senior staff. To successfully monitor, evaluate and review occupational therapy treatment/interventions within a multi-disciplinary framework and to modify as necessary taking a key worker role as indicated.
    10. Develop and conduct occupational therapy intervention with clients assessing their occupational needs and addressing quality of life, and physical, cognitive, affective, social, and environmental issues utilising appropriate individual or group treatment methods.
    11. To use developed physical skills in client interventions, such as home exercise programs, activities of daily living adaptation and training, positioning, palliative occupational therapy.
    12. To work autonomously in isolated community situations which vary in relation to local socio-economic and multi-cultural issues with telephone support available if required.
    13. To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care.
    14. To have the ability, knowledge and skills required for moving and handling people in community environments.
    15. To case manage clients effectively by undertaking daily handovers, ensuring timely and appropriate information is reported to the relevant professionals, making referrals etc.
    16. To update and monitor the safe staffing/acuity and dependency assessments of clients
    17. To support the service in meeting service staffing requirements by undertaken community visits or cover for Coordinator staff where necessary in order to ensure that a safe service is provided to clients.
    18. To be physically able to install specialist occupational therapy equipment in a client's home applying safe manual handling at all times, especially when doing so in restricted spaces.
    19. To ensure client and service records are recorded and maintained on IT databases and that client documentation is managed according to trust and local policies.
    20. To prescribe, delegate and monitor the work of Community Health Support Workers.
    21. To educate and teach the relevant health workers to participate in on-going reablement and care programmes for clients, as appropriate.
    22. To actively be involved in the development of new specialist clinical services within the team, with support and guidance from senior OT staff.

    Experience

    Essential criteria

    • Post-qualification experience in NHS or similar healthcare economy
    • Experience in independently managing a caseload of patients
    • Experience of supervising junior colleagues & students
    • Evidence of multi-disciplinary working

    Desirable criteria

    • Previous community therapist experience
    • Experience in a rehabilitation setting

    Ability & Skills

    • Develop and maintain communication with people about difficult matters and/or in difficult situations
    • Contribute towards improving quality in own work area and encourage others to do so
    • Support equality and diversity
    • Intermediate IT skills

    Desirable criteria

    • Sound decision making skills and the ability to prioritise own caseload & time management activity in the face of conflicting demands
    • Rehabilitation & disability management skills

    Knowledge

    Essential criteria

    • Knowledge and understanding of the NHS modernisation agenda and the implications for community services
    • Knowledge of long-term conditions including end of life care and the drive toward care for these patients

    Education/Qualifications

    Essential criteria

    • Degree/Diploma in Occupational Therapy or Physiotherapy
    • HCPC membership
    • Relevant Professional Body Membership
    • Evidence of continued professional development

    Desirable criteria

    • Post-graduate qualification relevant to the healthcare environment
