Role: Practice Pharmacist
We have an exciting opportunity for a Pharmacist to join our well-established, friendly and passionate team at Welbeck RoadHealth Centre.
We are a forward-thinking, training GP practice which covers approximately 11,500 patients. We also provide dispensing services to approximately 3,400 patients. We have a large, diverse multi-disciplinary team (including two Practice Pharmacists) and we are proud of our team ethos and patient care.
This is a new, permanent post where you will be able to make a significant difference to our patient population. We have a degree of flexibility with the amount of hours and also the working pattern of the individual.
The successful candidate will be a qualified Pharmacist, registered with the General Pharmaceutical Council. Previous experience of working in a GP Practice is desirable but not essential. In addition, being an independent prescriber is also desirable but not essential - we may be able to support the successful candidate through the Independent Prescribing Programme.
Main duties of the job
Providemedication review services to patients.
Presentand participate in internal/external meetings.
Undertakeface-to-face/telephone contact with patients, including patientmedication reviews, home visits may be required.
Withincompetencies, assess acutely-presenting patients.
Conduct long-term condition management where appropriate.
Manage and develop the prescription reauthorisation system and update patientmedications/discharge reviews and medicines reconciliation.
Whereneeded, undertake patient audits and reviews arising from national, local orpractice guidance.
Develop and manage a medicines management plan anddeliver patient services as determined by practice policy and local andnational guidance.
AssistPartners with the appropriate monitoring and management of prescribingbudgets.
Prepareevidence-based resources to support the medicines managementteam and other relevant health professionals in the implementation of cost-effective prescribing.
Helpplan, develop and support the introduction of new working processes within thepractice to optimise the quality of prescribing.
Develop protocols and SOPs.
Deal with medication-related queries from other team members, patients anddispensers.
Promotesafe prescribing and medicines management.

Welbeck Road Health Centre is a high achieving GP practice, sitting in the lovely town of Bolsover, Derbyshire with a branch surgery in the nearby village of Glapwell. Easy to access for many candidates with just a few minutes drive from junction 29A/M1, approximately a 30 minute drive from Nottingham and Sheffield and a 45 minute drive from Derby.
