To be responsible for the provision of a clinical service, working as part of the practice and PCN multidisciplinary team, delivering care within their scope of practice/PCN to the entitled patient population. The physician associate will be required to work autonomously providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure when required.

    Main duties of the job

    The following are the core responsibilities of the physician associate. There may be, on occasion, a requirement to carry out other tasks this will be dependent upon factors such as workload and staffing levels. The physician associate will work within their scope of clinical practice and:

    - Undertake face-to-face, telephone and online consultations for emergency or routine problems, as determined by the PCN, including management of patients with long-term conditions. Where required by the PCN, physician associates can offer specialised clinics following appropriate training including (but not limited to) family planning, baby checks, COPD, asthma, diabetes and anticoagulation.

    - Identify, signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients health

    - Undertake home visits and participate in duty rotas

    - Provide routine care to patients as required in accordance with clinical based evidence, NICE and the NSF

    - Examine, assess and diagnose patients & provide clinical care/ management as required

    - Triage patients & provide the necessary treatment during home visits g. Collect pathology specimens as required

    - Maintain accurate clinical records in conjunction with extant legislation

    - Ensure read codes are used effectively

    - Ensure continuity of care, arranging follow-up consultations or reviews as necessary

    -Chaperone patients where necessary

    - Prioritise health issues and intervene appropriately

    About us

    Greenwich PCN Alliance is formed of 4 PCNs across Greenwich, Blackheath and Charlton PCN, Eltham PCN, Heritage PCN and Unity PCN. A successful candidate will be allocated to one PCN and will work across that PCN only.

    Primary Responsibilities:

    - Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes

    - Chaperone patients where necessary

    - Prioritise health issues and intervene appropriately

    - Recognise, assess and refer patients presenting with mental health needs

    - Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

    - Liaise with external services/agencies to ensure that the patient is supported appropriately (vulnerable patients etc.)

    - Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times

    - Support the clinical team with all safeguarding matters, in accordance with local and national policies

    - Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

    - Deliver opportunistic health promotion where appropriate

    Secondary Responsibilties:

    In addition to the primary responsibilities, the physician associate may be requested to:

    - -Support the PCN audit programme, undertaking audits when necessary

    - -Support junior members of the team, providing guidance when necessary

    - -Participate in local initiatives to enhance service delivery and patient care

    - -Support and participate in shared learning within the practice

    - -Continually review clinical practices, responding to national policies and initiatives where appropriate

    - -Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA)

    - -Work in accordance with PCN policies, including the equal opportunities policy and practice.

    - -Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and the Quality and Outcomes Framework.

    - Work with due regard to the Health and Safety Policies across the PCN, ensuring that all practices and procedures are undertaken in accordance with issued guidelines

    - -Take personal responsibility for own learning and development, including the requirement to maintain currency, achieving all targets set in their Personal Development Plan (PDP)

    Induction training

    All personnel are to complete a practice induction programme. This is managed by the Business manager.

    Learning and Development

    The effective use of training and development is fundamental in ensuring that all staff are equipped with the appropriate skills, knowledge, attitude and competences to perform their role.All staff will be required to partake and complete mandatory training as directed by the training coordinator, as well as participating in the PCN training programmes.Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career and ultimately enable them to improve processes and service delivery.

    Collaborative working

    All staff are to recognise the significance of collaborative working.Team work is essential in multidisciplinary environments.Effective communication is essential and all staff must ensure that they communicate in a manner which enables the sharing of information in an appropriate manner.

    Service delivery

    Staff at Greenwich PCN Alliance must adhere to the information contained within practice/PCN policies and regional directives, ensuring that protocols are adhered to at all times.Staff will be given detailed information during the induction process regarding policy and procedure.

    Professional Conduct

    At Greenwich PCN Alliance, staff are required to dress appropriately for their role.Administrative staff will be provided with a uniform whilst clinical staff must dress in accordance with their role.

    Leave

    All personnel are entitled to take leave.Managers are to ensure all of their staff are afforded the opportunity to take a minimum of 25 days leave each year pro rata and should be encouraged to take all of their leave entitlement.
