    South Warwickshire Foundation Trust is committed to learning and development, and actively encourages people to make the most of their potential. We currently have vacancies for the Specialist Community Public Health Nursing (Health Visiting) course with practical placements in specified bases across Warwickshire. The course will start in September 2022 and will be offered in partnership with The University of Wolverhampton.

    We are seeking qualified degree level Nurses or Midwives with a current NMC registration, as this course is offered at Masters Level. In addition you will be able to demonstrate an awareness of evidence-based practice, government policy driving the public health agenda and collaborative working, as well as excellent interpersonal and leadership skills.

    Main duties of the job

    To engage in training in order to complete the BSc (Hons) Degree / Postgraduate Diploma in Specialist Community Public Health (Specialist Practitioner award in Health Visiting).

    This will involve liaising with all relevant parties within the practice area and at the assigned University in order to gain the desired 50% theory and 50% practical element of the course. In order to attain successful completion, the student will be required to work as part of a multi -disciplinary team within the primary care setting, carrying out clinical duties with clients as agreed with their Practice Teacher. The student will be accountable to the respective local manager according to local management structures, and responsible to the allocated Practice Teacher for the term of training.

    Patient Care Delivery / Communication

    Developing and maintaining effective communication networks with clients and other health professionals, statutory and voluntary agencies.

    Accurately and timely recording of all care given to the clients and report any changes in the clients condition to the health visiting team, practice teacher and the general practitioner or other member of the health / social care team as required.

    Attending and participate in staff meetings, case conferences and other meetings as required.

    Identifying problems with communication systems and helping to improve its effectiveness with other professionals and team members.

    Ability to provide comprehensive and accurate written records that may be used in enquiries and / or court sessions.

    Ability to formally present and discuss with individuals and groups ideas and issues pertinent to Health Visiting and Public Health.

    Being responsible for the assessment of care needs for identified case load / client group.

    Discussing and identifying who is responsible for the assessment of identified client groups.

    Identifying the multiple needs of the patient, family / carer, giving counsel and advice appropriate to the situation.

    Using the knowledge and skills necessary to assess individuals and groups in a holistic way taking into consideration of cultural differences.

    Promoting clients rights during the assessment process.

    Working within the primary care setting as a member of the health care team, participating in activities, which address the health needs of the caseload or client group.

    Providing and maintaining a high standard of care for clients in their homes, health centres, children centres and practice surgeries using a model of care, that is consistent with best evidence and within own scope of practice and legislation.

    Agreeing in partnership with the individual client or group in the planning of, implementation of, and evaluation of a robust care plans.

    Ensuring the changing needs of individuals and groups are identified in a timely manner and ensure adjustments to programmes of care are made.

    Identify, plan and undertake specific health promotional activities with identified target groups.

    Promoting and maintaining optimum health in clients / carers.

    Providing health education to all clients/ carers to include care and safety of drugs, equipment and prevention of accidents.

    Giving advice on the health needs of the community.

    Actively involving the target group in the planning and implementation of any proposed programme of intervention.

    Following local employing organisations policies and guidelines in all suspected forms of abuse.

    Ensuring that concerns and identified potential risks are referred to the appropriate Medical Practitioner and other related agencies immediately.

    The student will be expected to take responsibility for their adult learning and to inform their Practice Teacher and University contact should there be any exceptional circumstances preventing the completion of any aspects of the course.

    It is stressed that some of the information, which is handled by the student health visitors is of a highly confidential nature and should not be divulged to another party nor should it be discussed with colleagues unless it is essential to do so.

    The student will be expected to follow local organisations sickness and absence policy, reporting sickness or absences to both the employer and university.
