    Job summary

    An excellent opportunity has arisen for an enthusiastic and experienced frailty nurse to join our expanding clinical team at Parkbury House Surgery, on a part time basis for hours per week, although this is flexible.

    The team aims to deliver a service to individuals with complex and long-term health needs, to promote health and well-being, encourage and promote self-care and independence, while aiming to avoid unnecessary hospital admissions and attendances. We aim to deliver this service in practice, in the patients own home or in a care home setting, dependent on the patients needs.

    Main duties of the job

    As well as developing and maintaining clinical skills to a high standard you will be expected to have skills and experience in other areas essential to the role such as long term conditions, dementia and frailty, ideally you will also be a prescriber with a minor illness qualification.

    You will need to be able to assess, plan, implement and evaluate care for patients with long term or complex frailty needs, recognising individual physical and psychological needs and observing the principles of holistic care when planning treatment, which are sensitive to the ethnic, cultural and spiritual needs of the patient.

    You will be able to provide evidence based clinical care with measurable standards and outcomes, aiming to improve care and quality of life, with an awareness of up-to date local and national guidelines in subjects relevant to the role.

    You will be able to work as part of a team, supporting and complimenting the skills of the GPs, clinical pharmacists and other nurses to deliver a holistic service to our patients, while supporting and collaborating with the care team in the care homes and the wider multidisciplinary team who have an input into the care of our patients.

    About us

    Our practice operates from two sites with eight GP partners, seven salaried GPs, a large nursing team and over 40 non-clinical staff, providing care to over 21,000 patients. We are also a GP training practice.
