    Case Management and Intervention Lead

    About the Job Title:
    The position of Case Management and Intervention Lead focuses on providing support and guidance to vulnerable young people placed in a Secure Children's Home, aiming to inspire positive change and development.
    Leeds

    Job Description:
    - Lead on the case management and delivery of targeted intervention for a unit of 8 young people
    - Play a vital role in the progress, development, and transition opportunities for vulnerable young people
    - Inspire change in attitudes, behaviours, learning, and emotions among young individuals

    Requirements:
    - High level of communication skills to a range of individuals
    - Self-awareness and ability to reflect on personal practice
    - Confidence in communicating and liaising with stakeholders
    - Competency in making detailed assessments within the area of expertise

    Responsibilities:
    - Contribute proactively to the positive development of young people through high-quality assessment and planning
    - Assess individual needs and provide appropriate interventions
    - Develop effective working relationships with colleagues and external partner agencies
    - Coordinate and attend reviews for young people in a timely manner
