Job overview
We are looking for a motivated individual, preferably with previous experience of operational management (but not essential) to join the Older Peoples Division as Service Manager for the Derby City OA CMHT and Discharge Liaison Team.
The Derby City OA CMHT is based within St Andrews House and the Discharge Liaison Team is located at Kingsway Hospital.
There is a requirement to be visible at both team bases throughout the working week.
The CMHT is multi-disciplinary in nature, being made up of Nurses, OTs, Support Workers, Psychology, Medical colleagues and Administrators.
The Discharge Liaison Team is a small team, unique to the South of the county who undertake specialist placement / nursing assessments.
The role of Service Manager is to manage the daily functioning of the teams, ensuring that safe and effective care is delivered to the patients within the locality. It is a varied and busy position.
The successful individual will receive regular supervision, peer support, access to action learning sets and there will be development opportunities.
