    Main duties of the job

    Asa Senior Community Nurse you will be expected to work as a key member of themulti-disciplinary team to assist in reducing hospital admissions, facilitatinghospital discharges, delivering end of life care and community nursing. Yourrole will involve the assessment of patients holistic needs and theimplementation of robust care plans, alongside case management and allocation.You will be given management and supervisory responsibilities for band 3 and 5staff, offering them guidance and support.

    Thisis a great opportunity for personal and professional development that willexpand your knowledge and experience within the Integrated care System. Youwill be supported by the Band 7 Team Leader with regular supervision.

    Youwill be expected to work a combination of early (07:30 15:30), mid ( and late (10:30 18:30) shifts as well as weekends. We are happy toconsider flexible working requests.

    Weare a community-based neighbourhood teamproviding care to patients in their own home. We aim to deliver an effective,high quality service that puts patients at the centre of their own care. Youwill work closely with family, GPs, Social Care, the Voluntary and privatesector, Community Therapists, Advanced Clinical Practitioners, working acrossthe planned and anticipatory, admission prevention, hospital discharge and,rapid response pathways.

    Job responsibilities

    The successful candidate will:

    Workas an autonomous practitioner with the integrated care team providing rapid andplanned expert professional assessment and treatment interventions to patientsin their own homes or other community settings to enable patients to maximisetheir independence, health and wellbeing and remain in their own homes for aslong as possible. This will be achieved through a combination of hands onexpert clinical skills and knowledge to treat, educate and support patients ona designated caseload and by working with patients and their carers to assess,plan and implement appropriate packages of care.

    Demonstrateexpert clinical knowledge, health promotion and holistic assessment skills andwill work with the patient and their carers to assess, plan, implement andevaluate their ongoing care and support needs in their own home ortemporary/usual place of residence.

    Haveresponsibility for leading and managing a team of nurses within the locality.They will be responsible for the triage, allocation of work and the day-to-daymanagement of junior nursing staff and assistants as well as the standard ofclinical practice within the team.

    Thepost holder will support the Nurse Team leader in the dissemination of bestnursing practice, allocation and management of resources and will deputise inhis/her absence.

    Qualifications

    Essential

  • Registered Nurse (RN1)
  • Graduate Nursing Qualification at Academic Level 6 or working towards.
  • Preceptorship training
  • Evidence of ongoing continuing professional development
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer
  • Desirable

  • Specialist practitioner, District Nursing (degree/diploma) or equivalent level nursing qualification
  • Non medical prescriber (V150)
  • Mentor award /Practice Assessor
  • Evidence of ongoing continuing professional development
  • Experience

    Essential

  • Significant post registration experience in a clinical setting able to demonstrate transferability of experience to community setting
  • Experience of management of caseloads or wards and organisation of staff
  • Clinical audit/service improvement and development work
  • Experience of active participation in interdisciplinary team working
  • Desirable

  • Previous community nursing experience
  • Transcribing medication
  • Knowledge
