    Job summary

    This is an exciting post, based at the Royal Brompton Hospital and Wimpole Street, a leading centre for the care of patients with cardio-respiratory disease. The Private cardio-respiratory outpatient physiotherapy service is part of a growing, dynamic and motivated team. We provide excellent outpatient physiotherapy care to patients with a variety of conditions including non-CF bronchiectasis, asthma (omit severe), Primary Ciliary Dyskinesia, COPD, ILD, Cystic Fibrosis. A range of therapies are provided including airway clearance, nebulised drug response assessments, breathing re-education, assessment of long-term and ambulatory oxygen therapy, as well as individual cardiorespiratory rehabilitation.

    This post offers huge opportunity for service development and research in specialist services that are internationally recognised and also the opportunity for the post holder to develop extended scope skills such as non-medical prescribing and advanced airway clearance.     Main duties of the job

  • Undertake a highly specialist role under the guidance of the private patient clinical specialist Physiotherapist, assessing and treating patients with non-CF bronchiectasis, asthma, Primary Ciliary Dyskinesia, COPD, ILD, Cystic Fibrosis, as well as cardiorespiratory patients requiring rehabilitation.
  • Assessment for long-term and ambulatory oxygen therapy, making recommendations regarding appropriate equipment and devices.
  • Complete drug response assessments where necessary for patients requiring nebulised mucolytics/ antibiotics and liaise with all parties involved (medical teams/pharmacy).
  • To take a lead role in the advanced assessment and treatment of patients who may have highly complex needs, to determine clinical diagnosis and physiotherapy treatment indicated and maintain records as an autonomous practitioner.
  • To organise, co-ordinate and deliver the private Physiotherapy outpatient service day to day; alongside the PP clinical specialist, to utilise excellent communication and interpersonal skills to liaise with consultants and other stakeholders, both inside and outside of the organisation to develop the service.
  • To support the private patient clinical specialist Physiotherapist for in delivering high quality specialist assessment and physiotherapy treatments across the pathway, including maintaining financial records.
  • To play a major role in the continuous development of the service using audit, evaluation and quality improvement methodology.

    Job responsibilities

    Professional/ clinical responsibilities

  • To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care.
  • To ensure a high standard of clinical care for the patients under your management.
  • To interpret and analyse clinical and non-clinical facts to form accurate diagnosis and prognoses in a wide range of highly complex conditions, to recommend the best course of intervention, and to develop comprehensive discharge plans.
  • To undertake an advanced comprehensive assessment of patients, including those with a complex presentation, using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning, and utilising a wide range or treatment skills and options to formulate a specialised programme of care.
  • This role will require staff to routinely work across several sites including Royal Brompton Hospital and Wimpole Street, as well as Harefield Hospital.
  • To accept clinical responsibility for a designated specialised caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time.
  • To demonstrate highly developed dexterity, co-ordination and palpatory senses for assessment and manual treatment of patients.
  • To provide spontaneous and planned advice, teaching and instruction to relatives, carers, patients and other professionals to promote understanding of the aims of physiotherapy, and to ensure a consistent approach to patient care.
  • To give highly specialist advice, teaching and training to other members of the Multidisciplinary team (MDT) regarding the medical management of patients with cardio-respiratory problems.
  • To give specialist advice to the MDT and medical staff (to Consultant level) regarding physiotherapy techniques.
  • To communicate effectively with patients and carers to maximise Physiotherapy treatment/ rehabilitation potential and to ensure understanding of condition and physiotherapy involvement.
  • To be responsible for physiotherapy provision and private patient waiting list in conjunction with private patient clinical specialist physiotherapist.
  • To demonstrate highly developed time management skills.
  • To maintain accurate, comprehensive and up to date documentation, in line with legal and departmental requirements, and to communicate assessment and treatment results to the appropriate disciplines and patients in the form of reports and letters.
  • To be aware of health and safety aspects of your work and implement any policies, which may be required to improve the safety of your work area including your prompt recording and reporting of accidents to senior staff.
  • To carry out risk assessment within own patient caseload and environment on a daily basis (health & safety and clinical).
  • Service development

  • To work with other staff in the strategic and operational management of the service.
  • To undertake the measurement and evaluation of your work and current practices through the use of evidence-based practice, audit and outcome measures, either individually or with clinical specialist. Make recommendations for change.
  • To demonstrate a sound understanding of clinical governance and risk management and apply to work situations.
  • To undertake and involve physiotherapy staff in clinical research projects liaising with the Therapy Lead and the PP clinical specialist physiotherapist.
  • To decide priorities for own work area, balancing other patient related and professional demands, and ensure that these remain in accordance with those of the section as a whole.
  • To assist in optimum service delivery throughout the whole service, as demands require.
  • People management

  • To maintain own continued professional development (CPD) by keeping abreast of any new trends and developments and incorporate them as necessary into your work.
  • To be an active member of the in-service training programme by the attendance and delivery of presentations and training sessions at staff meetings, tutorials, training sessions in house and by attending and speaking at external courses/ conferences and practising reflective practice.
  • To participate in the staff appraisal scheme and personal development plan (PDP) as both appraiser and appraise whilst ensuring all staff actively participate in CPD.
  • Communication

  • To demonstrate an ability to communicate complex, emotive and sensitive information to patients, carers and other staff, where there may be barriers to communication or may include information about long-term / permanent disabilities ensuring confidentiality at all times
  • To clearly convey complex knowledge of techniques, biomechanics, anatomy and physiology to patients and staff, where evidence underpinning practice may be conflicting and ensuring sensitivity to the audiences level of understanding and prior knowledge
  • To be able to motivate and persuade others (including staff and patients) through advanced communication skills, with the benefit of verbal, non-verbal skills, using written and electronic information
  • To facilitate patients attitudinal change towards their condition thus encouraging health/function promoting behaviour
  • To articulate effectively the specialist physiotherapeutic perspective on a patients condition with medical colleagues and members of the multidisciplinary team, negotiating when various patient management options are available.
  • To diffuse potentially hostile and antagonistic situations with staff, patients and relatives, using highly developed negotiation and interpersonal skills
  • To use a wide variety of adult learning techniques to optimise clinical development
  • To represent the Rehab and Therapies Directorate within the Trust and to external agencies regularly, and on a district and national level occasionally
  • To support the operational leads in investigating and resolving written and verbal complaints and to be well versed with the Trusts formal complaints procedure within specialist area, in conjunction with the Rehabilitation and Therapies Leadership Team
  • To promote the role of Physiotherapy in own specialty within the Trust, local communities and nationally.
  • To cascade corporate strategic ideas and information to all staff within designated area, whilst being sensitive to their levels of understanding and prior knowledge
  • Information Management

  • To be responsible for maintenance of accurate records including appropriate electronic systems and supervision of departmental record keeping; to include comprehensive progress and discharge reports to medical referrers and legal and disability reports
  • To identify appropriate outcome measures and service impact measures which accurately evaluate patient response and service development needs
  • To maintain accurate statistical information on specialist area using databases as necessary to inform management team and drive audit agenda
  • Education and Professional Development.

  • To present service development/research regularly at local and occasionally national/international level.
  • To maintain a CPD portfolio reflecting personal professional development and ensure members within the specialty Physiotherapy team have a comprehensive performance plan
  • To keep abreast of physiotherapy evidenced based practice by use of relevant reading, attendance at in-service training, external courses and database searches.
  • To support in the development of local departmental standards based on a good working knowledge of National standards where they exist and on best evidence and to ensure these standards are monitored and adhered to.
  • To influence the National agenda in the development of standards and guidance in own specialist Physiotherapy service.
  • To promote a research environment within the specialist Physiotherapy team and offer support and guidance to research teams, MSc projects and external agencies using Trust property and patients and to identify sources of research funding where appropriate
  • As part of the Rehab and Therapies Directorates commitment to meeting patient needs and delivering excellence, physiotherapy services are provided 7 days a week. It is therefore essential the post holder be able to work their hours flexibly and contribute fully to both existing and new rotas.
