    Job summary

    An exciting opportunity for an experienced nurse to join the complex care team.

    The Complex Care team are looking for an enthusiastic registered nurse, with a strong clinical background with supporting children with complex health care needs to join our team. The successful candidate will need to have a passion for working with children and their families, whilst helping to promote independence and wellbeing for children and young people with Complex Care needs.

    Each day working in the complex care team is different, and our team members are always looking for ways to overcome the issues that we face, so the successful candidate must enjoy a challenge, be a dynamic thinker and problem solver.

    The successful candidate will be part of the clinical team, working independently managing their own case load and collaboratively with other key professionals whilst ensuring that they fulfil the ethos of looking after the whole child. They will have an opportunity to build cross agency relationships, whilst working closely with colleagues from Social care and Special Education Needs.

    Working closely with our admin team the successful candidate will plan and coordinate their own MDT meetings, including support the arrangement of emergency provision for children who are fast tracked to the team due to needing end of life care or experiencing mental health crisis.

    The successful candidate will complete assessments as part of the CCG delivery of the NHS Continuing Care framework

    Job responsibilities

  • To establish and monitor appropriate health care packages for children with continuing care needs
  • To use the Assess, Plan, Implement and Review model in conjunction with the child and family and other involved professionals, creating plans of care and protocols relevant to each child.
  • To participate in the negotiation of service level agreements with external providers.
  • To ensure that agencies contracted to deliver nursing care are maintaining their responsibility and adhering to agreed protocols.
  • Monitor agencies delivering care are meeting Key Performance Indicators and provide regular reports to the CCG.
  • To maintain up to date accurate records, provide written reports as needed and complete statistical returns as required.
  • To act in accordance with Trust policies in relation to Child Protection issues.
  • To manage risk and incidents, seeking advice and support managerially or clinically as appropriate.
  • To attend training courses as required and extend knowledge and skills in line with service requirements to maintain your NMC registration and revalidation.
  • To be aware of and act in accordance with agreed policy, procedures, legislation and NMC standards.
  • To work in partnership with social care and education teams, ensuring that holistic needs are met for children and families.
  • To undertake other duties and tasks as requested by the Community Matron for Childrens Complex and Continuing Care

  • To meet regularly with the Community Matron to discuss, plan and co-ordinate service development in area of responsibility.
  • To lead and/or participate in professional development/training as identified by clinicians or national directives.
  • To foster and develop effective working relationships within the service.
  • To contribute to the identification of the health needs of sick children and bring unmet needs to the attention of the Community Matron for Childrens Complex and Continuing Care.
  • To ensure financial transactions are carried out in accordance with the Trust's instruction.
  • To participate in the negotiation of service level agreements.
  • To contribute to clinical audit and encourage research based practice.
  • To participate in the recruitment, selection and appointment of nursing staff where appropriate.
  • To work collaboratively with Disabled Childrens Teams and CCG to ensure effective information sharing and planning respite or additional support/nursing care at home, ensuring an equitable service is offered to families.
  • To develop policies and guidelines for this clinical area ensuring they are communicated and evaluated to ensure a dynamic, safe and responsive environment for the child and family.
  • To facilitate transition to adult services when appropriate.
  • To monitor the stock of consumables, being mindful of the changing equipment needs of the client group

    Education/Qualifications

    Essential

  • RN Child or RSCN at degree level
  • NMC registration
  • Evidence of Continued Professional Development post Registration
  • Desirable

  • High Dependency Nursing Course
  • APLS
  • MSc level of study in paediatric physical assessment
  • Knowledge and Experience

    Essential

  • 5 years post registration experience within the paediatric setting
  • Knowledge and understanding of the Continuing care framework
  • Evidence of ability to maintain effective working relationships with colleagues, children & families
  • To be competent and experienced in ventilation and facial CPAP/BIPAP
  • Evidence of ability to provide clinical supervision and debriefs to junior staff.
  • Desirable

  • Experience of working within a community setting with complex needs children and young people
  • Evidence of clinical management experience
  • To be competent and experienced in tracheostomy
