    Job summary

    JOB RESPONSIBILITIES

  • See patients that are acutely unwell.
  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.
  • request pathology services as necessary.
  • Process and interpret pathology and other results that you have requested.
  • Recognise when patients need to be referred to secondary care and complete the referral.
  • Action any A&E documents or out of hours documents. be part of the duty doctor rota.
  • Main duties of the job

  • Manage patients presenting with a range of acute medical conditions.
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Maintain accurate clinical records in conjunction with extant legislation.
  • Prioritise health issues and intervene appropriately
  • Recognise, assess and refer patients presenting with mental health needs
  • Extended and supplementary prescriber, adhering to extant guidance
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
  • Contribute to practice targets (QOF etc.), complying with local and regional guidance
  • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies

    We are looking to recruit an PartTime Advanced Nurse Practitioner to work within our existing teams to deliver services to patients withminor illness or long term conditions. You will be working alongside Clinical Practitioners, Nurses and a wellsupportive team.

    We will be joining a Peak Edge Primary Care Network in April 2024 and are looking forward to be working with likeminded practices who share our vision. We are now in a position to recruit our PCN staff.

    This post will be providing care and treatment to Sloan Medical Centre patients only and based at the medical centre or branch site.

    We work hard as a team to support our patients but are always mindful to support each other. Building a strong relationship within our team also builds a strong support structure.

    We aim to deliver enhanced services to support all our patients needs and aim to increase the health and well-being of our population. To do this we will employ staff and find innovative and sustainable solutions to the changing needs of patients in a modern NHS.

    We are looking to recruit an Part Time Advanced Nurse Practitioner to work within our existing teams to deliver services to patients with minor illness or long term conditions. You will be working alongside Clinical Practitioners, Nurses and a well supportive team.

    We can offer the successful candidate the following;

  • Salary £ - £ (pro-rata and depending on experience)
  • Hours: 20 hours per week, over 3 days
  • NHS pension
  • Holidays: 6 weeks plus one week study pro-rata
  • Regular training and supervision
  • Weekly MDTs

    Qualifications

    Essential

  • Registered Nurse NMC
  • V300 prescribing qualification
  • Must hold a digital badge or able to obtain before start date
  • Experience
