    Main duties of the job

    Nursing Associates work in practice,independently under the leadership of registered nurses, working within thesphere of nursing and care, and within all aspects of the nursing process.

    They will support in taking Smears, Childs immunisations, bloods, ECG, Asthma and Diabetes reviews.

    Job description

    Job responsibilities

    . Toassist and perform routine nursing tasks related to patient care, such asroutine BP checks, ECG, suture and clip removal, BMI and lifestyle advice, asdirected and agreed with line manager, registered general nurses and GPs.

    To support patients to improve and maintain theirmental, physical, behavioural health and wellbeing by using behaviour changeinterventions, such as undertaking health checks (physical and mental), smokingcessation services.

    Tosupport the RGN with the holistic assessment and planning of wound care plansfor patients with chronic and acute wounds.

    Toimplement agreed wound care plans, following local and national evidence-basedguidelines, inclusive of the treatment of minor traumatic wounds within ownlevel of competency. To recognise any deterioration of wounds or infection andarrange early review with PN/GP or specialist as required.

    Carryout Chronic Disease Management annual reviews delegated by the Practice Nurseor senior colleague.

    Supportthe Practice Nurse with the management of patients with long-termconditions/chronic diseases, using a holistic patient centred approach.

    Assistin and perform routine tasks related to patient care as directed by thePractice Nurse GP or other member of the Primary HealthCare Team.

    Beable to undertake baseline observations and identify signs of improvement,report any deteriorations or concerns, pulse, blood pressure,respirations, oxygen saturations, peak flow, spirometry, blood glucose levels,ECG, assessing nutrition and hydration, participate in screening programmes andAnnual Reviews for example in patients with Learning Disabilities .

    Performcervical smear screening on completion of additional training as guidance RCNPosition Statement on Registered Nursing Associates (RNAs) training in cervicalsample taking.

    Administermedications as prescribed and in accordance with NMC standards vianebuliser or injections using sub-cutaneous or intramuscular route underPatient Specific Directions.

    Followingagreed protocols with referral to Practice Nurse or GPs as appropriate assistin the assessment, planning, development implementation and evaluation ofprogrammes to promote health and well-being and prevent adverse effects onhealth and well-being.

    Deliveropportunistic health promotion using opportunities such as new patient medicalsand NHS health checks.

    Documentconsultations accurately and appropriately, according to NMC guideline

    Ensureawareness of statuary and local clinical adult and children safeguardingprotection procedures, including systems of referral. Ability to recognisesigns and symptoms of abuse/neglect.

    Ensureinfection prevention and control guidelines are maintained.

    Tobe responsible for providing the appropriate nursing evidenced based care of anallocated group of patients.

    Bean accountable professional who is responsible for their actions, working inline with NMC standards of proficiency.

    Recogniseand work within the limits of their competency.

    Towork as part of multidisciplinary team using resources efficiently.

    Todeliver quality care to patients and to work effectively within the nursingteam.

    Tobe competent in carrying out a variety of clinical procedures.
