    Job summary

    The post holder will be an essential part of the Home Engagement and Rehabilitiation Team. The team offers intensive support to a small caseload of individuals with complex mental health needs (and often with complex co-morbidities too)

    The successful candidate will work as part of MDT team in leading/supporting on physical health clinics with support and supervision from physical health nurse.

    The clinics will be to support service users under the team with their physical and mental wellbeing.

    Main duties of the job

  • To work in an active role as part of the MDT
  • To contribute to zoning, MDT meetings, CPA reviews and assessments
  • To contribute to the assessment and monitoring of service users mental health and wellbeing
  • Completion of Physical Health Improvement Tool (PHIT) for all service users.
  • Liaison with other primary and secondary health services
  • Health Promotion
  • Undertake Venepuncture as required
  • Undertake ECG as required.
  • Supporting annual flu vaccination clinics

    Job responsibilities

  • The post holder will play an active role as a member of the multidisciplinary team. The post holder will participate and contribute to multi-disciplinary reviews, care programme approach meetings and other clinical meetings communicating assessments and delivery of care as part of the overarching care package.
  • The post holder will have daily access to information of a confidential nature and will be expected to comply with NHS guidelines and Trust Information Governance policies on confidentiality and information sharing, and ensure communication systems are used appropriately, written communication, systems.
  • The post holder will have responsibilities for collaborating and communicating regularly and effectively with service users carers, families and other agencies involved in care. This will require the person to manage and engage with sensitive and sometimes, distressing information.
  • The post holder will demonstrate empathy and positive regard for the people who access our services. They will also be able to adapt to barriers in communication that service users may present with emotional, psychological and physical, utilising a high level of expertise in communication skills.
  • The post holder will have responsibilities for communicating both written and verbal information to the team on a daily basis, and the ability to assess, deliver and monitor care and communicate this effectively within clinical case notes of service users under the supervision of a registered nurse.
  • The post holder will be expected to establish and maintain effective working relationships with the clinical team, wider multidisciplinary team and other agencies acting professionally.
  • Act upon any changes or concerns in the condition of a service user, immediately reporting any issues to a line manager or duty worker

    Qualificationjs/Education

    Essential

  • Registered Nurse Associate compliant with the requirements for NMC registration Level 5 study, Foundation Degree Nursing Associate. End Point Assessment achieved as passed. Training in: Venepuncture, BLS, ECG taking,Awareness of blood results relevant to role. willingness to undertake training in all of the above is essential

    Essential

  • Assessing physical health needs. Awareness of mental health and physical health and how it interacts. Have the ability to work on their own initiative under appropriate supervision and engage in reflective practice .Have experience of delivering person centred care under the supervision of a RN. Have experience of working in collaboration with members of a multidisciplinary team in delivering care

  • Have previously completed a placement or have been employed in a community mental health setting .Have experience of working with people with psychosis

  • The post holder should have a working knowledge and understanding of: Evidence based interventions and their application on practice. The scope of the role of Nursing Associate in the context of the team and the organisation, and how the role may contribute to service development. When to seek advice and refer to a registered care professional. The need for continued professional development in accordance with the requirements of NMC registration

  • Knowledge of specialist needs of service user group substance misuse/self-harm. Knowledge of PHIT. Knowledge around smoking cessation. Awareness of Mental Health Act and Mental Capacity Act as relevant to role.

Able to complete appropriate records, documentation .Able to display and use empathy, compassion and patience. Able to form and maintain positive therapeutic relationships with service users and carers. A keenness to make a positive contribution to improving the quality of life of service users and carers. Ability to act calmly in emergencies and to respond in a professional manner to stressful and challenging behaviour. Experience of undertaking physical health observations. Assessment of physical healthcare using Early Warning Scores and appropriate escalation
