    Description

    An exciting opportunity has arisen within the Colorectal/StomaCare Nursing Dept at Wye Valley NHS Trust for a full time Band 6 nurse to join our friendly and supportive team for a 12 month secondment or fixed term contract.


    At Wye Valley NHS Trust the Colorectal/Stoma Care Nursing Team provide holistic support to patients and their families and carers on theirColorectal pathway.

    As part of the Multi-disciplinary team the successful post holder will be instrumental in helping to develop new pathways aspart of an evolving Service in order to meet the needs of our patients both in the acute and community settings.

    They will be required to work autonomously and as part of a team and will need to be flexible and resilient, demonstrating sensitivity to others in sometimes distressing situations.

    The successful candidate will have opportunity to enhance their professional development within an experienced and professional team.

    As a Colorectal/Stoma Care Nurse you will be expected to:o Provide specialist Colorectal/Stoma care advice and support to patients, their families and carers and other healthcare providersthroughout the patients care pathway.

    o Be an active participant in already established Nurse Led pathways and will be offered additional training in order to support thiselement of the role.o Work in collaboration with Primary care to provide a community based stoma nurse service facilitating the integration of this servicewithin the WVT Colorectal/Stoma Care work plan.o Participate in the education and training of patients, family and carers and other health care providers within the speciality.o Make use of audit and research data to evaluate the service and ensure that best evidence based care is provided.o Be actively involved in health promotion and prevention strategies and adhere to Wye Valley Trust policies and procedures.
