    Planning & Commissioning Officer - Nottingham, United Kingdom - National Care Group

    National Care Group (NCG) is a leading provider of care and support services to vulnerable adults throughout the United Kingdom.

    We work in partnership with the people we support, their family members, local authorities and care providers to provide high quality, person-centred support across the following services: supported living, residential care, outreach & home support, and day services.

    We are currently recruiting for a Care Plan Reviewing Officer to join our team.

    The role will involve quality checking the accuracy of care and support plans, audits and risk assessments and care system information across National Care Group services in line with national regulatory standards in England & Wales.

    The postholder will report to and also support the Chief Risk and Quality Officer to monitor overall quality and compliance across all National Care Group Services and inform the implementation of quality improvement initiatives.

    The ideal candidate will have experience of working in Social Care sector preferably with adults and young people.

    Candidates must have a proven track record in social care and or governance, reviewing social care or health packages and good IT skills.

    Review and assess individual care plans to ensure they are comprehensive, person-centred, and in compliance with regulatory requirements and company standards.

    ensure that information is consistent across all care documentation through triangulation of records and ensure that all documents accurately reflect the needs of the people supported.

    to ensure that information provided has been captured and detailed within care documentation by the local managers and there is evidence that actions have been delivered and evidenced.

    Collaborate with operational, commercial and wider stakeholders to conduct thorough assessments of a person's needs, preferences, and goals to inform the development of a detailed care plans.

    Analyse documentation related to an individual's care, including assessments, progress notes, and medication records, to identify any discrepancies or areas for improvement.

    ensure that local teams are inputting information regarding care provision in an accurate and contemporaneous manner into digital care systems and the detail is thorough enough to fully evidence care delivery and risks.

    Offer constructive feedback and guidance to colleagues regarding the development and implementation of support plans, ensuring alignment with best practices and an individual's needs.

    Monitor adherence to care plans and regulatory requirements through regular audits and reviews, identifying opportunities for enhancement and ensuring continuous quality improvement.

    Work closely with operational, commercial, healthcare providers, and other stakeholders to promote effective communication, coordination of care, and interdisciplinary collaboration.

    Develop and deliver training programs and educational materials to colleagues in understanding and implementing best practices related to care planning and documentation.

    Maintain accurate and up-to-date records of audit findings, recommendations, project plans and corrective actions taken to ensure accountability and transparency.

    Bachelor's degree in nursing, Social Work, Healthcare Administration, or related field (preferred).
    Proven experience in a social care or healthcare setting, preferably in care planning, quality assurance, or auditing roles.

    Strong understanding of regulatory requirements and standards related to care planning and documentation (e.g., CQC, Ofsted, CIW or CI).

    Proficiency in Microsoft Office Suite and electronic health record (EHR) systems.
    Commitment to upholding ethical standards and promoting person-centred care principles.
    Experience at an operational level within a compliance, contract monitoring or commissioning role.
    Experience in a care reviewing role or similar experience.
    Experience of working in the field of learning disabilities, mental health or autism
    NVQ 4/5 in Care or similar qualification
