    Band 7 Community Matron - 37.North East London Foundation Trust have an exciting opportunity for a Community Matron in Waltham Forest, ICT Central , Integrated Community Team.

    Community Nursing are the beating heart of our health and care services - we look forward to you working with us .

    You will be providing case management, facilitating early discharge from hospital and preventing unnecessary/unplanned hospital admissions where appropriate as well as supporting healthcare closer to home.

    00hrs, Monday to Friday.

    The post holder will be a Registered Nurse, with at least 2 years community experience of managing a complex caseload of patients.

    You will be responsible for a caseload of patients with complex health and social care needs.

    You will work autonomously to case manage these clients and use advanced nursing skills, such as assessment and prescribing to manage their conditions in the community.

    Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

    All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.

    As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes.

    The induction will be held at our head office in Rainham, Essex.

    The Community Matron :

    Will utilize a range of quality improvement tools to review professional practice and initiate and evaluate changes to practice when required.

    Will access Clinical Supervision and professional support from sources agreed with the line manager in order to support critical reflection and support safe practice.

    Will support and supervise colleagues and provide targeted education, training and development as required.

    Will engage positively in performance management and participate in the review and development of their Personal Development Plan As required.

    Will demonstrate effective clinical and professional leadership in regards to developing the strategy for the development of services surrounding Long Term Conditions
    ability to understand and interpret clinical data and information
    evidence of NHS plan, frameworks and government policies
    basic awareness of IT and IT skills
    experience with managing patients with long term conditions
