    Job Overview

    The post holder will manage both the human and financial resources of a nursing team under the supervision of a named District Nurse Team Manager. They will be accountable for the allocation of patient care to nurses within the District Nursing Team and for supervising their work. Additionally, they will be responsible for assessing, planning, implementing, and evaluating patient care, ensuring professional and clinical standards are met. The successful candidate will act as a liaison between the community and hospital to promote integration and continuity of care, working unsocial hours as required.

    Main Duties of the Job

    Managing Care

    • To effectively and efficiently deliver nursing care to patients delegated by District Nurse Team Manager.
    • To undertake skilled nursing activities related to patient care assessment, planning, and evaluation, as well as supervising and training junior staff and students.
    • To promote personal, professional, and clinical expertise through continuous practice evaluation and updating.
    • To ensure practice aligns with the NMC code and upholds the 6Cs, including compassionate care, as a service ethos.
    • To assist in leading the team to fulfill the service's mission statement and integrate it into practice.
    • To collaborate with various departments and agencies for integrated working and care.
    • To engage in health promotion, including chronic disease management, for patients and families.
    • To identify a trained nurse within the team as the named nurse for patients.
    • To participate in care/case management under the District Nurse Team Manager's supervision.

    Main Duties

    Managing Care

    • Responsibility for delivering nursing care efficiently as delegated.
    • Skilled nursing activities for patient care assessment, planning, and evaluation.
    • Continuous professional development and practice updates.
    • Adherence to professional codes and promotion of service ethos.
    • Leading the team to achieve the service's mission.
    • Liaising with various departments and agencies for integrated working and care.
    • Promotion of health initiatives and disease management.
    • Assigning a named nurse within the team for patient care.
    • Participation in care/case management under supervision.

    Managing Resources

    To oversee team clinical practice, identify training needs, and ensure resources reflect caseload health needs. Promote communication, employment practices, and team leadership to achieve service objectives.

    To manage resources in alignment with team responsibilities and compliance standards. Uphold financial guidelines and utilize technology for reporting and information sharing.

    Record Keeping

    Ensure accurate records are maintained for patient care safety and continuity, along with submitting statistical data promptly as per policy.

    Quality

    Participate in policy setting, quality initiatives, and clinical audits. Adhere to clinical governance standards and promptly address service quality issues. Support team development through supervision and training.

    Professional Development and Responsibilities

    Share expertise with staff, participate in training programs, projects, and health promotion initiatives as needed.

    Research

    Engage in research projects and contribute to nursing team and community health service development through collaboration and participation in various groups.

    Person Specification

    Education/Qualifications

    Essential Criteria
    • RN 1st level registration
    • Evidence of recent CPD
    • Demonstrated ability in clinic nursing settings
    Desirable Criteria
    • Additional postgraduate certification

    Experience

    Essential Criteria
    • Teamwork and integrated working experience
    • Understanding and application of 6Cs in practice
    • Knowledge of Clinical Governance and supervision
    • Caseload prioritization and resource management
