    Job summary

    An opportunity has become available for an experienced PCN Manager with experience in general practice and strong business intelligence & IT skills to join our Celandine Health & Metrocare PCN.

    Main duties of the job

    The role of the PCN Manager ispivotal in supporting the delivery of Primary Health Care across Primary Care Network(PCN) through promoting collaborative working of member practices to providejoined up care.

    The PCN Manager is responsible for ensuring that both PCNactivities are safely run on a day-to-day basis,incorporategood governance, and supports practices to achieve their objectives. Thepost-holder will ensure that the PCN operatein a risk averse, legal, and cost-effective manner, in keeping with thefinancial aspirations and running of the PCN. The role will require agileworking, in multiple locations including regular office presence and willinclude remote working. You may need to work across the PCN practice sites sofrequent short journeys may be required as well as attending evening meetingsas and when the need arises.

    You will be supported by the PCN representatives, ClinicalDirectors, our admin support, Lead Practice managers andTransformation manager.

    Job responsibilities

  • Work with the PCN CD(s) and member practices to facilitate delivery of the requirements of the PCN DESs, including working to specified timelines. This will include support with communications with Commissioners, answering queries, and pursuing claims.
  • Become fully conversant and knowledgeable on the documentation produced by NHS England on the PCN DES and lead on submitting documentation to the CCG.
  • Act as a contact point for clinicians who lead for certain areas of the DES, including Structured Medication reviews, Enhanced Health in Care Homes and Early Cancer Diagnosis and future priorities.
  • Lead on Contract Management for the PCN.
  • Support the Clinical Director to ensure the smooth running of the Network.
  • Coordination with the other Hillingdon PCN Managers for the benefit of all PCNs.
  • Optimising benefit from specialist resources in terms of IT, BI, transformation, training and education, finance, HR and similar functions by collaborating with all Hillingdon PCNs and the Confederation over their supply.
  • Co-ordinate the delivery of enhanced services and other service submissions on behalf of the Network.
  • Document and manage PCN workflow processes as they develop.
  • Lead and manage the PCN development plan and work with PCN subject matter leads such as IT and communication to deliver a cohesive joint plan.
  • Be involved in the management and development of the PCN.
  • Ensure the PCN has a Business Continuity policy and other key policies that are updated and in which everyone is kept fully aware.
  • Develop strategic and operational planning processes to support the PCNs objectives.
  • Write business and development plans as required.
  • To work with The Confederation Transformation Team to support and implement projects and change.
  • Ensure the PCNs compliance with the GDPR and FOI along with other relevant legislation professional information management standards such as GMC/BMA and LMC guidance, oversee the administration of information management and technology of the PCN, and ensure the integrity of the system working with the CCG to implement their guidance.
  • Maintain a conflict of interests register for member practices.
  • Maintaining Data Sharing Agreements for the PCN.
  • Write policies, procedures and protocols that would apply to the PCN to ensure there is clear guidance, transparency and governance in place.
  • Ensure that policies and procedures are in place to minimise the risk of NHS fraud.
  • Ensure the highest standards of governance and probity in all duties.
  • Develop PCN protocols and procedures, review and update as required.
  • Act as a central source of financial information for the PCN.
  • Engaging with practices, supporting them to work together to be an effective PCN.
  • First point of contact for Practice Managers with regard to delivery of Network projects/schemes.
  • Promote effective communication within the Network.
  • To engage and communicate with the ICS partners.
  • To acknowledge and confirm that all information will be cascaded to the necessary members of staff.
  • To collaborate with their PCN staff to help develop effective communications.
  • Contribute to communications to help these be deliverable to the correct target audience.
  • Edit and distribute content.
  • To establish and maintain effective relationships with other communication leads to help deliver the valuable communications. Contribute to communications being uploaded via social media (provide content). Ensure that all communications are reaching appropriate staff and provide feedback to aid engagement.
  • Communicate effectively with other team members the post holder is to recognise the significance of collaborative working. Teamwork is essential in a multidisciplinary environment. Communication is essential and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate way.
  • Communicate effectively and professionally with all colleagues and external bodies you come into contact with.
  • Recognise peoples needs for alternative methods of communication and respond accordingly.
  • Have an established method in effectively capturing patient feedback and using the information to help improve service delivery.
  • Working and effective PPG which contributes to designing and improving services.
  • Support recruitment, induction and training of new PCN staff as required.
  • Manage the recruitment process for staff being employed onto the Additional Roles Reimbursement scheme. This may involve working with The Confederation, Hillingdon CIC, who may act as the employer for some or all of these roles. This will include coordinating appraisal and clinical supervision requirements.
  • Coordinate and link into The Confederation Training hub for support and guidance and development of the PCN workforce for new and established roles.
  • Supporting Training hubs in establishing PCN learning environments.
  • Organise PCN events Protected Learning Time/Educational Events.
  • Co-ordinate training and development of clinical/administrative staff where related to PCN activities.
  • Responsible for developing and coordinating a skills matrix of existing workforce.
  • Identifying the gaps and training needs for the workforce.
  • Training and development plans created for upskilling workforce which builds PCNs resilience.
  • Ensuring PCN has robust financial management frameworks, policies and process which are within a financial management framework established with policies and procedures adhered to by all PCN members.
  • To be responsible for the production of performance and quality reports within the PCN and other Network initiatives including accurate monthly reports produced for PCN.
  • To be able to produce budget forecasting for services and overall PCN funding for budget, actual spend, forecasting and balance.
  • To be responsible for the set up and oversight of the Networks financial systems/payments to all the practices within the Network.
  • To monitor and check the accuracy of the any submissions for Enhanced Services, Contractual requirements on behalf of the Network and ensure that all claims are submitted on a monthly and quarterly basis.
  • Manage contractual requirements on behalf of the Network, ensure that the PCNs physical and financial resources are managed and maintained for maximum effectiveness through:
  • Monitoring expenditure and identifying significant deviations from plan.
  • Planning and monitoring income and cash flow to ensure that income is maximised and that funds are available as required.
  • Ensuring all claims are submitted in good time and payment made and received, Ensuring the practices allocation of resources if fair and equitable.
  • Present financial plans, monitoring information and reports to the PCN as required.
  • Ensure the PCNs compliance with the GDPR and FOI along with other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance.
  • Request, monitor and send any authorised invoices to the Confederation for payment.
  • Liaise with the Confederation on any outstanding invoices that require payment.
  • Monitor overall PCN income and expenditure, ensuring cost effectiveness and probity.
  • Liaise with member practices to ensure there is representation and engagement from all practices at any meetings and keep a register of all attendances.
  • Ensure the practices/business have appropriate insurance cover/indemnity etc. for any PCN services that may be run from their premises.
  • To identify areas within the PCN that can be developed as a shared resource and/or streamline back office function.
  • Identify Quality Improvement projects to deliver on the PCN priorities.
  • Work with The Confederation Transformation team in supporting and developing projects.
  • To be responsible for the production of performance and quality reports within the PCN.
  • Feedback / analyse data on behalf of the Network to report to the CCG, Ensure that the PCN board and Clinical Director adhere to the governance structure as set out in the Network Agreement.
  • To be able to configure EMIS to support PCN working.
  • To standardise EMIS service processes, documentation, change control for practices within the PCNs.
  • To identify a trained cohort of PCN staff to be able to set up PCN services and maintain EMIS functions.
  • Responsible for cascading and embed learning.
  • Service redesign and process mapping to support service delivery.
  • Information governance support for PCN developed services.
  • Ongoing on site and remote support.
  • Demonstrate sound knowledge of NHS Governance and regulatory requirements
