    This opportunity is to work for two practices in Alpha PCN. Gossoms End Surgery in Berkhamsted and the Rothschild House Group in Tring. The working pattern is open to discussion but likely to be three long days at Gossoms End, Berkhamsted and one long day at Rothschild House Group, Tring.

    First round of interviews will take place via Teams Meeting within two weeks of closing date.

    Direct patient contact through assessment, examination, investigation, diagnosis and treatment

    Support practices with structured care around patients with long term conditions

    First point of contact of care to establish a working diagnosis and management plan in partnership with the patient

    Have direct and immediate access when needed to a GP and work under their supervision. Review, analyse and action diagnostic test results

    Deliver integrated patient centred-care through appropriate working with the wider primary care MDT

    Undertake consultations for emergency or routine problems and management of patients with long-term conditions

    Undertake home visits, participate in duty rotas and possess a valid driving licence

    Utilise clinical guidelines and partake in clinical audits, significant event reviews and other research and analysis tasks

    Keep up to date with evidence-based knowledge and competence in all aspects, meeting governance guidelines for CPD

    Main duties of the job

    A Post Graduate Degree in Physician Associate Studies from an accredited PA Programme

    Membership of the Faculty of Physician Associates at the RCP, which includes UK Physician Associate Registration (the PA Managed Voluntary Register)

    Current and valid certification or re-certification by the UK Physician Associate National Examination

    Experience of working in a medicine environment, including preventative medicine and chronic disease management

    Flexible and adaptable approach to a varied and high-volume workload with the ability to multi-task and to work under pressure

    Consistently compassionate and respectful approach placing a high value on the dignity of patients, relatives, carers and colleagues.

    Ability to communicate effectively with colleagues, patients, relatives, nurses and other staff and agencies

    Commitment to continuing professional development

    On-going training / study leave will be considered to support personal development.

    About us

    Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited (DHPL), are developing new and exciting additional roles under the 2020 scheme, to support GPs and clinical teams at enhancing patient services in the local community.

    There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. The role will contribute to improving quality of patient care across the network of practices.

    Job responsibilities

    Responsible For:

    The responsibilities of the role include direct patient contact through assessment, examination, investigation, diagnosis and treatment. You will maintain a key role in supporting delivery of network contracted services and to provide a holistic clinical service, with support from GPs as required.

    Job Purpose:

    To help support practices with structured care around patients with long term conditions, including frailty.

    Provide first point of contact of care for patients presenting with undifferentiated, undiagnosed problems utilising history-taking, physical examinations and clinical decision-

    making skills to establish a working diagnosis and management plan in partnership with the patient (and their carers, where applicable).

    Will have direct and immediate access when needed to a GP/Doctor and work under supervision of the GP with which the patient is registered.

    Review, analyse and action diagnostic test results.

    Deliver integrated patient centred-care through appropriate working with the wider primary care multi-disciplinary team and social care networks.

    Undertake face-to-face, telephone and online consultations for emergency or routine problems, as determined by the PCN, including management of patients with long-term conditions. Where required by the PCN, offer specialised clinics following appropriate training including (but not limited to) family planning, baby checks, COPD, asthma, diabetes and anticoagulation.

    Undertake home visits and participate in duty rotas.

    Must possess a valid driving licence.

    Provide health/disease promotion and prevention advice to patients.

    Utilise clinical guidelines and promote evidence-based practice and partake in clinical audits, significant event reviews and other research and analysis tasks.

    Through participating in continuing professional development opportunities, keep up to date with evidence-based knowledge and competence in all aspects of the role, meeting clinical governance guidelines for continuing professional development (CPD).

    Develop and agree a personal development plan (PDP) utilising a reflective approach to practice, operating under appropriate clinical supervision.

    On-going training / study leave will be considered to support personal development.

    Manual Handling

    The post holder will be provided with adequate training in correct lifting techniques by a recognised lifting instructor.

    • Current and valid certification or re-certification by the UK Physician Associate National Examination
    • Maintains CPD requirements of 50 hours per year (25 Type 1 and 25 Type 2)
