Discharge to Assess/ Rehab Team
Are you an experienced Band 5/6 physiotherapist, with strong leadership qualities and assessment skills, seeking a new and diverse challenge?
West Lothian continues to invest in delivering community assessment and rehabilitation closer to home through our Home First programme. This programme has seen significant investment in our community AHP teams and has recently secured funding to introduce a community single point of contact to enhance the patient pathways.
The successful applicant would be joining a very stable and dynamic multi- disciplinary team with a passion for integrated working putting patient outcomes and experience at the heart of their practice. Including working closely with our inpatient colleagues to identify and facilitate early supported discharge and provide rehabilitation in the community.
We are a forward thinking and high performing Partnership promoting excellence and innovation and providing safe and compassionate support to our patients within West Lothian. This means being a provider of excellent patient safety, putting the patient at the heart of all decisions and supporting them in the right setting by the right skilled professional at the right time to optimize their outcomes.
This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
Please note: As this post is part-time the salary will be pro-rata.
Flexible Hours /Part Time Hours will be considered pending the outcome of the interview.
NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
