    Lead Pharmacist for Community Services - Salford, Greater Manchester, United Kingdom - Northern Care Alliance NHS Foundation Trust

    Description

    To lead, develop, evaluate, and formulate long term strategic plans to ensure a consistently high-quality clinical pharmacy and medicines optimisation service to the Intestinal Failure directorate at Salford Royal Hospital.

    To work with the multidisciplinary teams to optimise patient outcomes and improve medicines safety. To lead on the teaching and education of medicines optimisation in Intestinal Failure across the pharmacy department.

    As a member of a National Reference Centre for Intestinal Failure, to provide teaching and education at a regional and national level.

    Provides a specialised clinical pharmacy service to patients under the IntestinalFailure Directorate as part of a multidisciplinary team.

    To oversee the clinical pharmacy service to the Intestinal Failure Directorate andto lead on relevant medicines optimisation and safety initiatives within thespecialty.

    To plan and implement, under the direction of the Principal Clinical Pharmacist,the strategic direction of the clinical pharmacy service provided.

    Undertakes risk management and ensures compliance with Medicines legislationincluding national and local policies.

    Lead on relevant Intestinal Failure or general clinical training/ teaching/ educationsessions for pharmacists under the direction of the Principal Clinical Pharmacist.

    Provides out of hours emergency pharmacy services including bank holidays,weekends, lates and on-call.

    To form part of the Senior Clinical Pharmacy Leadership and Management teamand provide (along with other senior pharmacists) specified operational supportto the pharmacy department.

    Vocational master's degree in pharmacy or equivalent
    Post Grad Diploma / MSc in Clinical Pharmacy
    Certification of non-medical Prescribing

    Management Qualification

    Registered with the General Pharmaceutical Council

    Knowledge, Training & Experience
    Extensive professional knowledge acquired through vocational master's degree in pharmacy
    Pre-registration training and experience
    Extensive post qualification clinical pharmacy experience
    Commitment to continuing professional development / education
    Extensive experience, knowledge, and practical clinical pharmacy skills within Intestinal Failure
    Good time management skills
    Skills for analysing drug charts and complex patient, medicines, or financial information in order to provide advice on medicines, dosages, and expenditure
